OPERATING PERMITS


Intent to Issue Title V Operating Permits under the Air Pollution Control Act and 25 Pa. Code Chapter 127, Subchapter G.

   Southeast Region: Air Quality Program, Lee Park, Suite 6010, 555 North Lane, Conshohocken, PA 19428; Thomas McGinley, New Source Review Chief, (610) 832-6242.

   46-00013: Hatfield Quality Meats, Inc. (2700 Funks Road, Hatfield, PA 19440) for a renewal of their Title V Operating Permit in Hatfield Township, Montgomery County. The initial permit was issued on December 29, 1998. The facility is a hog rendering plant. As a result of potential emissions of NOx, PM and SOx, the facility is a major stationary source as defined in Title I, Part D of the Clean Air Act Amendments and is therefore subject to the Title V permitting requirements adopted in 25 Pa. Code Chapter 127, Subchapter G. The proposed Title V Operating Permit Renewal includes one new boiler subject to 40 CFR 60, Subpart Dc, several small (new) combustion sources and a reclassification of the existing cold parts washers to comply with new State regulations. At this time, the facility is not subject to Compliance Assurance Monitoring under 40 CFR Part 64. The renewal contains all applicable requirements including monitoring, recordkeeping and reporting.

   33-00033: Owens-Brockway Glass Container, Inc.--Plant 18 (Cherry Street, Brockway, PA 15824) for reissuance of a Title V Permit to operate a glass containers manufacturing facility in Brockway Borough, Jefferson County. The facility's major emission sources include glass melting furnace, refiner, two forehearths, two lehrs, eight raw material handling and silos, cullet handling and storage, batch gathering and mixing, miscellaneous combustion sources, two container forming machines, road fugitives and 100 kW emergency generator and two degreaser units. The facility is major facility for Title V due to its potential to emit NOx and SOx.

   10-00023: Mercer Lime and Stone Co.--Branchton Plant (560 Branchton Road, Slippery Rock, PA 16057) for a Title V Operating Permit reissuance to operate a lime manufacturing facility in Slippery Rock Township, Butler County.

The permit will include monitoring, recordkeeping and reporting requirements designed to keep the facility operating within all applicable air quality requirements.

   46-00118: Ursinus College (P. O. Box 1000, Collegeville, PA 19426) for a non-Title V Facility, State-only, Synthetic Minor Operating Permit in Collegeville Borough, Montgomery County. The permit is for the operation of 52 boilers and 12 emergency generators firing natural gas, no. 4 or no. 6 oil as fuel. The permit will include monitoring, recordkeeping, reporting and work practice standards designed to keep the facility operating within all applicable air quality requirements.

   46-00179: Tuscan-Lehigh Dairies LP (880 Allentown Road, Lansdale, PA 19446) for operation of two boilers at their dairy bottling plant in Upper Gwynedd Township, Montgomery County. The permit is for a non-Title V (State-only) facility. The facility has the potential to emit less than 25 tons of VOCs and less than 25 tons per year of NOx; therefore the facility is a Natural Minor. The permit will include monitoring, recordkeeping and reporting requirements designed to keep the facility operating within all applicable air quality requirements.

   67-03010: Yorkraft Inc. (550 South Pine Street, York, PA 17405) for operation of their two spray booths and four UV coaters in York City, York County. The facility has a potential to emit 12.2 tons per year of VOCs. The State-only operating permit will contain appropriate conditions designed to keep the facility operating below Title V thresholds for criteria pollutants and within all other applicable air quality requirements.

   06-03057: Borough of Bally (P. O. Box 187, Bally, PA 19503) for the operation of a dual stripping tower at their facility in the Borough of Bally, Berks County. The State-only operating permit will include limits on the concentration of certain contaminants. The permit will include testing, monitoring, work practices, recordkeeping and reporting requirements designed to keep the facility operating within all applicable air quality requirements.

   06-05079: United Corrstack, LLP (720 Laurel Street, Reading, PA 19602) for production of roll stock paper from recycled paper at its facility in the City of Reading, Berks County. The State-only operating permit will limit emissions to levels below the Title V permit thresholds. The permit will contain emission limits, monitoring, recordkeeping and reporting requirements to ensure the facility complies with the applicable air quality regulations.

   36-03108: Bristolpipe (88 Newport Road, Leola, PA 17540) for operation of a plastic pipe manufacturing facility in Upper Leacock Township, Lancaster County. The State-only operating permit shall contain testing, monitoring, recordkeeping and reporting requirements, emission restrictions and work practice standards designed to keep the facility operating within all applicable air quality requirements.

   36-03019: Flury Foundry Company (1160 Elizabeth Avenue, Harrisburg, PA 17601) for the operation of a brass and bronze foundry in Manheim Township, Lancaster County. Annual emissions from this facility are expected to be about 4.0 tons of PM10 per year. The State-only operating permit will include monitoring, recordkeeping, reporting requirements, emission restrictions and work practice standards designed to keep the facility operating within all applicable air quality requirements.

   25-00591: Seaway Manufacturing Corp. (2250 East 33rd Street, Erie, PA 16510) for a Natural Minor operating permit for the manufacture of vinyl and metal doors and windows in the City of Erie, Erie County.

COAL AND NONCOAL MINING ACTIVITY APPLICATIONS

   Applications under the Surface Mining Conservation and Reclamation Act (52 P. S. §§ 1396.1--1396.19a); the Noncoal Surface Mining Conservation and Reclamation Act (52 P. S. §§ 3301--3326); The Clean Streams Law (35 P. S. §§ 691.1--691.1001); the Coal Refuse Disposal Control Act (52 P. S. §§ 30.51--30.66); and The Bituminous Mine Subsidence and Land Conservation Act (52 P. S. §§ 1406.1--1406.21). Mining activity permits issued in response to applications will also address the applicable permitting requirements of the following statutes: the Air Pollution Control Act (35 P. S. §§ 4001--4015); the Dam Safety and Encroachments Act (32 P. S. §§ 693.1--693.27); and the Solid Waste Management Act (35 P. S. §§ 6018.101--6018.1003).

   The following permit applications to conduct mining activities have been received by the Department of Environmental Protection (Department). A copy of an application is available for inspection at the district mining office indicated before an application. Where a 401 Water Quality Certification is needed for any aspect of a particular proposed mining activity, the submittal of the permit application will serve as the request for certification.

   Written comments, objections or requests for informal conferences on applications may be submitted by any person or any officer or head of any Federal, State or local government agency or authority to the Department at the district mining office indicated before an application within 30 days of this publication, or within 30 days after the last publication of the applicant's newspaper advertisement, as provided by 25 Pa. Code §§ 77.121--77.123 and 86.31--86.34.

   Where any of the mining activities listed will have discharges of wastewater to streams, the Department will incorporate NPDES permits into the mining activity permits issued in response to these applications. NPDES permits will contain, at a minimum, technology-based effluent limitations as identified in this notice for the respective coal and noncoal applications. In addition, more restrictive effluent limitations, restrictions on discharge volume or restrictions on the extent of mining which may occur will be incorporated into a mining activity permit, when necessary, for compliance with water quality standards (in accordance with 25 Pa. Code Chapters 93 and 95). Persons or agencies who have requested review of NPDES permit requirements for a particular mining activity within the previously mentioned public comment period will be provided with a 30-day period to review and submit comments on the requirements.

   Written comments or objections should contain the name, address and telephone number of the person submitting comments or objections; the application number; and a statement of sufficient detail to inform the Department on the basis of comment or objection and relevant facts upon which it is based. Requests for an informal conference must contain the name, address and telephone number of requestor; the application number; a brief summary of the issues to be raised by the requestor at the conference; and a statement whether the requestor wishes to have the conference conducted in the locality of the proposed mining activities.

Permit renewal application for continued operation and reclamation of an existing noncoal mine in East Franklin Township, Armstrong County, affecting 310 acres. Receiving streams: unnamed tributaries to the Allegheny River (WWF). The first potable water supply intake within 10 miles downstream from the point of discharge is West Penn Water Company. Renewal application received January 12, 2004.

FEDERAL WATER POLLUTION CONTROL ACT, SECTION 401

   The following permit applications, requests for Environmental Assessment approval and requests for 401 Water Quality Certification have been received by the Department of Environmental Protection (Department). Section 401 of the Federal Water Pollution Control Act (FWPCA) (33 U.S.C.A. § 1341) requires the State to certify that the involved projects will not violate the applicable provisions of sections 301--303, 306 and 307 of the FWPCA (33 U.S.C.A. §§ 1311--1313, 1316 and 1317) as well as relevant State requirements. Persons objecting to approval of a request for certification under section 401 of the FWPCA or to the issuance of a Dam Permit, Water Obstruction and Encroachment Permit or the approval of an Environmental Assessment must submit comments, suggestions or objections within 30 days of the date of this notice, as well as questions, to the regional office noted before the application. Comments should contain the name, address and telephone number of the person commenting, identification of the certification request to which the comments or objections are addressed and a concise statement of comments, objections or suggestions including the relevant facts upon which they are based.

   The Department may conduct a fact-finding hearing or an informal conference in response to comments if deemed necessary. Individuals will be notified, in writing, of the time and place of a scheduled hearing or conference concerning the certification request to which the comment, objection or suggestion relates. Maps, drawings and other data pertinent to the certification request are available for inspection between 8 a.m. and 4 p.m. on each working day at the regional office noted before the application.

   To: (1)  install and maintain a box culvert; (2) remove a multiple pipe culvert crossing and stabilize the area; and (3) construct and maintain a 5,430 ft2 building addition which will extend into the floodway of Deep Run (WWF), for the purpose of expanding operations at the Bell & Evans, Farmer's Pride Chicken Plant, 154 West Main Street, just north of Route 343 (Fredricksburg, PA Quadrangle N:  13.0 inches; W:  8.3 inches) in Bethel Township, Lebanon County.

   E36-776: Office of Lancaster County Engineer, 50 North Duke Street, 7th Floor, Lancaster, PA 17608 in Ephrata Township, Lancaster County, ACOE Baltimore District.

   To install and maintain a dual span concrete arch bridge structure a total of 66.3 feet in length, across Cocalico Creek (WWF), for the purpose of replacing the existing deteriorating bridge on Royer Road (T-648) approximately 2,250 feet northwest of Rothsville Road (Ephrata, PA Quadrangle N:  8.0 inches; W:  13.0 inches) in Ephrata Township, Lancaster County.

   E12-146. Steve Troncone, 585 East 2nd Street, Emporium, PA 15834. Troncone Gravel Removal in Shippen Township, Cameron County, ACOE Baltimore District (Cameron, PA Quadrangle N:  22.25 inches; W:  11.5 inches).

   The applicant proposes to remove gravel deposition within the Driftwood Branch of the Sinnemahoning Creek. The extent of the work will have a maximum length of the 150 linear feet and a maximum width of 30 linear feet to a depth not to exceed 6 inches above the water surface.

   E12-147. Lumber Township, 213 Rocky Road, Emporium, PA 15834. Hunts Run Road Project in Lumber Township, Cameron County, ACOE Baltimore District (Cameron, PA Quadrangle N:  17.95 inches; W:  3.5 inches and Wharton, PA Quadrangle N:  2.6 inches; W:  16.6 inches, N:  4.6 inches; W:  15.25 inches, N:  5.3 inches; W:  15 inches).

   The applicant proposes to replace either undersized or failing culvert crossings in four different locations along Hunts Run Road. Each of these locations has different drainage areas thus requiring different designs for each. The location information follows this order: White Head Run Stream Crossing proposes to remove the existing three 48-inch corrugated metal culvert pipes and replace them with five 60-inch culvert pipes, remove the existing culvert assembly in Colbert Hollow and replace it with two 60-inch culvert pipes, remove the existing 27-inch corrugated metal culvert pipe in an unnamed tributary to Hunts Run and replace it with two 48-inch culvert pipes and to remove the existing 24-inch corrugated metal culvert pipe in an unnamed tributary to Hunts Run and replace it with two 48-inch culvert pipes. The permit application labels the culverts in the unnamed tributaries to Hunts Run as culvert 2 and culvert 1, respectively. This permit proposes to impact a total of 125 feet of stream and does not impact any wetlands.

   E57-103. Department of Conservation and Natural Resources, Rachel Carson Office Building, P. O. Box 8451, Harrisburg, PA 17105-8451. Bridge replacement in Laporte Township, Sullivan County, ACOE Baltimore District (Eagles Mere, PA Quadrangle N:  12.8 inches; W:  4.2 inches).

   To: (1) remove the existing single-span steel bridge which is skewed 75 degrees to the roadway baseline with stone masonry abutments. The existing bridge has a normal span of 28.0 feet, a curb-to-curb width of 13.5 feet, a minimum underclearance of 3.82 feet and a hydraulic opening of 146 square feet; (2) construct and maintain a prestressed concrete spread box beam bridge which is skewed 60 degrees to the roadway baseline, with a single clear span of 35.0 feet, minimum underclearance of 6.2 feet, a curb-to-curb width of 18.0 feet, a normal span of 30.0 feet, a minimum underclearance of 6.2 feet and a hydraulic opening of 185 square feet. R-6 rock will be placed around the superstructure and along the far upstream and downstream channel banks for scour protection; and (3) construct a two-phase temporary diversion system consisting of a dike constructed of precast barrier, polyethylene liner and sandbags. The dike will initially divert water from the near abutment area and then be relocated to divert water from the far abutment area. The project is in Shanersburg Run (HQ-CWF) on Shanersburg Road approximately 0.5 mile from the intersection of Shanersburg Road with SR 154. The project will not impact wetland while impacting approximately 100 feet of waterway and less than 1 acre of earth.

   The applicant proposes to replace the existing right abutment of an existing bridge. The work includes removal of the existing right abutment and replacing it with a cast-in-place concrete abutment with railroad tie wing walls. The project will impact a maximum of 40 linear feet of Crooked Creek (WWF) and does not impact any wetlands.

The applicant proposes to place fill in 0.02 acre of de minimis area of wetlands associated with unnamed tributaries to Piersons Run (TSF) for the purpose of constructing The Whispering Pines Residential Development. The project is approximately 2,000 feet west from the intersection of Lindsey Lane and Old Frankstown Road and will impact 0.02 acre of wetlands and 934.0 linear feet of stream channel.

   E02-1447. Department of Transportation, District 11-0, 45 Thoms Run Road, Bridgeville, PA 15017. West End Bridge improvements in the City of Pittsburgh, Allegheny County, Pittsburgh ACOE District (Project begins: Pittsburgh West, PA Quadrangle N:  12.7 inches; W:  3.8 inches and ends N:  10.6 inches; W:  4.0 inches) (Latitude: 40° 26` 21"--Longitude: 80° 1` 58"). Applicant proposes to: (1) remove existing structures; (2) construct and maintain the new Steuben Street Connector Bridge, consisting of a single-span beam bridge with a clear span of approximately 91 feet and minimum underclearance of approximately 29.4 feet; (3) to construct and maintain a replacement South Street Bridge, consisting of s single-span beam bridge with a clear span of approximately 42 feet and minimum underclearance of approximately 18 feet; (4) to construct and maintain retaining wall approximately 220 feet long along the right bank; (5) to operate and maintain the existing Ramp A bridge consisting of a concrete arch culvert having a span of approximately 52 feet and underclearance of 29 feet; (6) to construct and maintain two temporary stream crossings; and (7) various stormwater outfalls, all of these structures are in or along or across Saw Mill Run (WWF). The work is part of the proposed West End Bypass Improvement Project--Phase 2. The project begins at the south end of the West End Bridge and ends near the intersection of the West End Bypass and Shaler Street in the City of Pittsburgh, Allegheny County. The project proposes to directly effect, through retaining walls 220 feet of stream channel and through bridges, approximately 250 feet of stream channel. No vegetated wetlands will be impacted.

   E03-417. Allegheny Energy Supply Company, LLC, 4350 Northern Pike, Monroeville, PA 15146-2841. Limestone Run Abandoned Mine Reclamation Wetland Fill in Washington Township, Armstrong County, Pittsburgh ACOE District (East Brady, PA Quadrangle N:  6.5 inches; W:  6.0 inches) (Latitude: 40° 54` 10"--Longitude: 79° 32` 30"). The applicant proposes to place and maintain fill in 3.05 acres of wetlands (PEM) as part of the reclamation of an abandoned strip mine. To compensate for loss of wetlands, the applicant proposes the construction of 3.4 acres of replacement wetlands onsite. The project is off of T-577, approximately 1.5 miles from its intersection with SR 4017.

   E04-300. Calaco, Inc., 307 4th Avenue, Suite 604, Pittsburgh, PA 15222. Pine Run Townhouse culvert and wetland fill in New Sewickley Township, Beaver County, Pittsburgh ACOE District (Baden, PA Quadrangle N:  11.6 inches; W:  15.4 inches) (Latitude: 40° 41` 20"--Longitude: 80° 14` 10"). The applicant proposes to construct and maintain a dual 60-inch diameter HDPE culvert for a distance of approximately 76 within a tributary to Crows Run (WWF) with 6 feet of riprap on the upstream and downstream ends, to place and maintain fill for a distance of approximately 400 feet along the right bank flood way of Crows Run, to construct and maintain various stormwater outfalls to Crows Run and to place and maintain fill in 0.02 acre (PEM). This is a de minimis impact, therefore no wetland mitigation is required. The encroachments are for a proposed development to be known as Pine Run Townhouses. The project is on the west side of Pine Run Road, approximately 1,000 feet north of its intersection with 9th Street. The total proposed impacts are 76-foot culvert in Crows Run, approximately 400 feet of fill in the floodway and 0.02 acre of wetland fill. This permit application was originally published under E04-292, which was withdrawn on July 1, 2003.

   E26-314. North Union Township Supervisors, 7 South Evans Station Road, Lemont Furnace, PA 15401. Nell Street (T-993) Culvert over Bennington Spring Run in North Union Township, Fayette County, Pittsburgh ACOE District (Uniontown, PA Quadrangle N:  0.5 inch; W:  10.7 inches) (Latitude: 39° 52` 38"--Longitude: 79° 41` 59"). The applicant proposes to remove the existing structure and to construct and maintain a 32.0-foot long bottomless 13.0-foot by 3.0-foot aluminum box culvert in Bennington Spring Run (WWF) for the purpose of improving transportation safety and roadway standards. The project is on T-993 (Nell Street).

   E30-208. Borough of Waynesburg, 90 East High Street, Waynesburg, PA 15370. Waynesburg Borough Municipal Park improvements in Waynesburg Borough, Greene County, Pittsburgh ACOE District (Waynesburg, PA Quadrangle N:  3.8 inches; W:  7.8 inches) (Latitude: 39° 53` 15--Longitude: 80° 10` 18"). The applicant proposes to improve and maintain the existing Waynesburg Borough Municipal Park by adding parking, a driveway, sidewalk, play ground equipment and picnic pavilion in the 100-year floodplain of Purman Run (WWF). The park is between High and Greene Streets.

   E63-556. Maronda Homes, Inc., 202 Park West Drive, Pittsburgh, PA 15275. Walnut Ridge Culvert in North Strabane Township, Washington County, Pittsburgh ACOE District (Washington East, PA Quadrangle N:  21.2 inches; W:  9.5 inches) (Latitude: 40° 14` 30"--Longitude: 80° 11` 53"). The applicant proposes to remove the existing driveway crossing, to construct and maintain a new 84-inch diameter RCCP culvert (depressed 6 inches) approximately 110 feet long and a riprap energy dissipater for 46 feet on the downstream end of the culvert, to place and maintain fill along the left bank flood plain for approximately 300 feet and construct and maintain a stormwater outfall to, in and along an unnamed tributary to Chartiers Creek (WWF) for a proposed residential development to be known as Walnut Ridge. The project will be on the south side of SR 519 approximately 0.6 mile east of its southbound exit off of SR 79 in North Strabane Township, Washington County. The total proposed stream impacts are 185 feet to an unnamed tributary to Chartiers Creek.

   E25-682, Department of Transportation, District 1-0, 255 Elm Street, P. O. Box 398, Oil City, PA 16301. SR 0020, Section 11M Across tributaries to Turkey Creek in Springfield Township, Erie County, ACOE Pittsburgh District (East Springfield, PA Quadrangle N:  13.6 inches; W:  15.8 inches).

   To conduct the following activities in tributaries to Turkey Creek on SR 0020, Section 11M just east of its intersection with SR 5:

   1.  To remove the existing structure and to install and maintain a 124-foot long, 76-inch wide by 48-inch high elliptical reinforced concrete pipe stream enclosure approximately 400 feet east of SR 5.

   2.  To remove the existing structure and to install and maintain twin 180-foot long, 48-inch diameter reinforced concrete pipe stream enclosure approximately 700 feet east of SR 5.

   Major Amendment E37-147, Department of Transportation, District 11-0, 45 Thoms Run Road Bridgeville, PA 15017-2853. SR 2007, Section L01, Segment 0040, Offset 0000 Stream Enclosure Replacement in an unnamed tributary to Slippery Rock Creek (CWF) in Perry Township, Lawrence County, ACOE Pittsburgh District (Portersville, PA Quadrangle N:  3 inches; W:  12.5 inches).

   The original permit giving its consent to remove the existing structure and to construct and maintain a 120-foot long, 12-foot wide by 6.5-foot high precast concrete pipe arch culvert stream enclosure is to be amended to give consent to remove the existing structure and to construct and maintain a 120-foot long, 12-foot wide by 6.5-foot high precast concrete box culvert in an unnamed tributary to Slippery Rock Creek (CWF, perennial) on SR 2007, Section L01, Segment 0040, Offset 0000 approximately 0.95 mile northwest of the intersection of SR 2007 (Armstrong Road) and SR 488. The project will directly affect a total of 120 linear feet of stream channel.

ENVIRONMENTAL ASSESSMENTS

   EA32-006. Robindale Energy Services, Inc., 7 Riverside Plaza, Blossburg, PA 16912. Dilltown Mine Reclamation Project in Brushvalley Township, Indiana County, Pittsburgh ACOE District (New Florence, PA Quadrangle N:  19.65 inches; W:  4.08 inches).

   The applicant proposes to place and maintain fill in a reported 0.45 acre of wetland (PEM), 200 linear feet of an unnamed tributary within the Blacklick Creek Watershed (CWF) and 0.09 acre of open water in association with a mine reclamation project. The project will fill three abandoned mine surface pits and eliminate approximately 4,150 feet of highwall with beneficial use flyash, soil placement, grading and seeding. The project will convert these areas into unmanaged grassland. The entrance road to the site is off of SR 0056, approximately 4 miles north of its intersection with SR 0022

   Project proposes to construct, operate and maintain a nonjurisdictional dam (detention basin no. 2) and to modify an existing nonjurisdictional dam (detention basin no. 1). The dams are across a tributary to Simpson Run in Monroeville Borough, Allegheny County. Detention basin no. 2 will impact approximately 100 linear feet of stream channel for stormwater management purposes. The embankment of detention basin no. 1 will be lowered so the reservoir will have a maximum depth of 14.27 feet. The dams are approximately 3,300 feet southwest of the intersection of Saunders Road (SR 2100) and Meadowbrook Road (Murrysville, PA Quadrangle N:  4.95 inches, W:  15.95 inches).

   D43-009EA. Borough of Grove City, P. O. Box 110, Grove City, PA 16127. Pine Township, Mercer County, ACOE Pittsburgh District.

   Project proposes to breach and remove an unnamed dam across Wolf Creek (CWF) to eliminate a threat to public safety and to restore the stream to a free flowing condition. The dam is directly under SR 208 approximately 500 feet east of the intersection of SR 208 and SR 173 (Grove City, PA Quadrangle N:  7.6 inches; W:  11.0 inches).

   D54-043EA. Schuylkill County Municipal Authority, 221 South Centre Street, P. O. Box 960, Pottsville, PA 17901, Tremont Township, Schuylkill County. AOE Baltimore District.

   Project proposes to breach and remove two unnamed dams across Poplar Creek (CWF) to eliminate a threat to public safety and to restore the stream to a free flowing condition. The dams are on the property of the Schuylkill County Municipal Authority approximately 3/4 mile southwest of the intersection of U. S. Route 209 and SR 125 (Pine Grove, PA Quadrangle N:  22.3 inches; W:  3.9 inches).

FINAL ACTIONS TAKEN FOR NATIONAL POLLUTION DISCHARGE ELIMINATION SYSTEM (NPDES) PERMITS AND WATER QUALITY MANAGEMENT (WQM) PERMITS

   The Department of Environmental Protection (Department) has taken the following actions on previously received applications for new, amended and renewed NPDES and WQM permits, applications for permit waivers and Notices of Intent (NOI) for coverage under general permits. This notice is provided in accordance with 25 Pa. Code Chapters 91 and 92 and 40 CFR Part 122, implementing provisions of The Clean Streams Law (35 P. S. §§ 691.1--691.1001) and the Federal Clean Water Act.

Section VII contains notices for parties who have submitted NOIs for coverage under general NPDES permits. The approval for coverage under general NPDES permits is subject to applicable effluent limitations, monitoring, reporting requirements and other conditions set forth in each general permit. The approval of coverage for land application of sewage sludge or residential septage under applicable general permit is subject to pollutant limitations, pathogen and vector attraction reduction requirements, operational standards, general requirements, management practices and other conditions set forth in the respective permit. Permits and related documents, effluent limitations, permitting requirements and other information are on file and may be inspected and arrangements made for copying at the contact office noted before the action.

   Persons aggrieved by an action may appeal, under section 4 of the Environmental Hearing Board Act (35 P. S. § 7514) and 2 Pa.C.S. §§ 501--508 and 701--704 (relating to the Administrative Agency Law), to the Environmental Hearing Board, Second Floor, Rachel Carson State Office Building, 400 Market Street, P. O.
