Title V Operating Permit No. 55-00002

   Under 25 Pa. Code § 127.521, notice is given that the Department of Environmental Protection (Department) intends to issue a Title V Operating Permit to the Selinsgrove Center State Hospital. Selinsgrove Center is located in Penn Township, Snyder County. The Selinsgrove Center representative to contact regarding this application is Hugh Grimes, Facility Maintenance Manager, P. O.    Selinsgrove Center operates three anthracite/bituminous coal-fired boilers to produce steam for heating the facility. The Tioga station is located in Farmington Township, Tioga County. The CNG Corporation representative to contact regarding this application is Sam S.    The Tioga plant is a natural gas transmission station. Electralloy is located in Oil City Township, Venango County. MINING ACTIVITY APPLICATIONS

   Applications under the Surface Mining Conservation and Reclamation Act (52 P. S. §§ 1396.1--1396.19a); the Noncoal Surface Mining Conservation and Reclamation Act (52 P. S. §§ 3301--3326); The Clean Streams Law (35 P. S. §§ 691.1--691.1001); the Coal Refuse Disposal Control Act (52 P. S. §§ 30.51--30.66); The Bituminous Mine Subsidence and Land Conservation Act (52 P. S. §§ 1406.1--1406.21). Mining activity permits issued in response to such applications will also address the applicable permitting requirements of the following statutes: the Air Pollution Control Act (35 P. S. §§ 4001--4015); the Dam Safety and Encroachments Act (32 P. S. §§ 693.1--693.27); and the Solid Waste Management Act (35 P. S. §§ 6018.101--6018.1003).

   The following permit applications to conduct mining activities have been received by the Department of Environmental Protection (Department). A copy of the application is available for inspection at the District mining office indicated above each application. Where a 401 water quality certification is needed for any aspect of a particular proposed mining activity, the submittal of the permit application will serve as the request for such certification.

   Written comments or objections, or requests for informal conferences on applications, may be submitted by any person or any officer or head of any Federal, State or local government agency or authority to the Department at the same address within 30 days of this publication, or within 30 days after the last publication of the applicant's newspaper advertisement, as provided by 25 Pa. Code §§ 77.121--77.123 and 86.31--86.34 (relating to public notices of filing of permit applications, opportunity for comment, and informal conferences).

   Where any of the mining activities listed will have discharges of wastewater to streams, the Department will incorporate NPDES permits into the mining activity permits issued in response to these applications. The NPDES permits will contain, at a minimum, technology-based effluent limitations (as described in the Department's regulations--25 Pa. Code §§ 77.522, 87.102, 88.92, 88.187, 88.242, 89.52, and 90.102) for iron, manganese, suspended solids, settleable solids, alkalinity, and pH. In addition to the above, more restrictive effluent limitations, restrictions on discharge volume, or restrictions on the extent of mining which may occur will be incorporated into a mining activity permit when necessary for compliance with water quality standards (in accordance with 25 Pa. Code Chapters 93 and 95). Persons or agencies which have requested review of the NPDES permit requirements for a particular mining activity within the above-mentioned public comment period will be provided with a 30-day period to review and submit comments on those requirements.

   Written comments or objections should contain the name, address and telephone number of persons submitting comments or objections; application number; and a statement of sufficient detail to inform the Department on the basis of comment or objection and relevant facts upon which it is based. Requests for an informal conference must contain the name, address and telephone number of requestor; application number; a brief summary of the issues to be raised by the requestor at the conference; and a statement whether the requestor desires to have the conference conducted in the locality of the proposed mining activities.

ENCROACHMENTS

   The following Dam Safety and Encroachment permit applications, requests for Environmental Assessment approval, and requests for water quality certification have been received by the Department of Environmental Protection. Section 401(a) of the Federal Water Pollution Control Act (33 U.S.C.A. § 1341(a)) requires the State to certify that the involved projects will not violate the applicable provisions of 33 U.S.C.A. §§ 1311--1313, 1316 and 1317, as well as relevant State requirements. Initial requests for 401 certification will be published concurrently with the permit application. To remove approximately 100 feet of gravel and silt deposits for a depth of approximately 2 feet and an average width of 13 feet in Stewards Run behind his residence on Cement Hollow Road for the purpose of flood control.    E41-408.  Encroachment. Williamsport Sanitary Authority, 253 West 4th St., Williamsport, PA 17701. Repair, operate and maintain an existing stream enclosure to carry 16 city streets across Grafius Run and to abandon an existing structure and to construct, operate and maintain an 18 inch sanitary sewer line to convey municipal wastewater across Grafius Run. The work shall consist of repairing the existing 10.0 feet × 10.0 feet concrete box culvert that encloses Grafius Run with a 3,750.0 linear section and installing an 18 inch diameter ductile iron pipe across Grafius Run. The project is located approximately 500.0 feet west of the intersection of Elmira Street and Hawthorne Street (Cogan Station, PA Quadrangle N: 0.20 inch; W: 1.6 inches) in the City of Williamsport, Lycoming County.    E41-409.  Encroachment. Pa. Dept. of Transportation, 715 Jordan Ave., Montoursville, PA 17754. To repair, operate and maintain the streambed, abutment footings and wingwall footings of the single span bridge that carries SR 184, Seg. 0100/000 across Wendell Run. The work shall consist of repairing the bridge abutment and wingwall scour by paving the natural streambed with reinforced concrete that shall have a maximum thickness of 6 inches. The project is located along the eastern right-of-way of SR 0287 approximately 2.7 miles east of the intersection of SR 184 and SR 4014 (White Pine, PA Quadrangle N: 7.4 inches; W: 9.5 inches) in Cogan Station Township, Lycoming County.    E53-299.  Encroachment. Pa. Dept. of Transportation, P. O. Box 342, Clearfield, PA 16830. To remove the existing structures and to construct and maintain two road crossings that will carry SR 4023, Seg. 120/1857 and Seg. 130/0000 across Elevenmile Creek. The two road crossings shall be constructed with corrugated metal culvert pipes that have a span of 29 inches and rise of 42 inches and a length of 16 feet. The project is located along the eastern right-of-way SR 044 approximately 4,000.0 feet east of the intersection of T-380 and SR 4023 (Oswayo, PA Quadrangle N: 15.5 inches; W: 3.3 inches) in Oswayo Township, Potter County. To remove the existing concrete box culvert with dimensions; 12 feet wide by 6 feet high by 50 feet long and to construct and maintain a multiplate steel arch culvert with dimensions 20 feet, 1 inch wide by 7 feet 6 inches high by 100 feet long in a tributary to Brush Creek (WWF). This project will also include a de minimis wetland impact for a commercial development. The project is located on the west side of S. R. 0019 approximately 3,000 feet south of the intersection of S. R. 0019 and S. R. 0228 (Mars, PA Quadrangle N: 9.3 inches; W: 14.0 inches) located in Cranberry Township, Butler County.

   E25-554.  Encroachment. Millcreek Township, 3608 West 26th Street, Erie, PA 16506. To repair damages to three existing streambank retaining walls with a total length of approximately 255 linear feet, to construct and maintain a new streambank retaining wall measuring 85 linear feet adjacent to an existing wall which will be repaired, to construct and maintain a 12-foot wide by 10-foot high by 193-foot long precast concrete arch stream enclosure in Beaver Run (CWF, MF), and to stabilize streambanks and slopes by grading and seeding. The project is located along Beaver Run from the S. R. 0019 (Peach Street) Bridge downstream for a distance of approximately 600 feet. This project will stabilize a portion of Beaver Run that has been damaged by failure of previously constructed streambank retaining walls (Erie South, PA Quadrangle N: 13.4 inches; W: 12.9 inches) located in Millcreek Township, Erie County.

   E27-056.  Encroachment. Pa. Dept. of Transportation, Engineering District 1-0, 1140 Liberty Street, Franklin, PA 16323. To remove the existing steel stringer bridge and to construct and maintain a 14-foot wide by 8-foot high by 40- foot long reinforced box culvert on S. R. 1003 across Slater Run (WQ-CWF). This box culvert will be placed approximately one foot below existing streambed elevation to allow for unobstructed access for aquatic organisms. This project will also include a temporary de minimis wetland impact associated with the site development.    E04-238-A2.  Encroachment. Connoquenessing Associates, 617 Portersville Rd., Ellwood City, PA 16117. To amend Permit No. E04-238 to place and maintain fill in an additional 0.01 acre of wetlands (PEM) and to maintain fill in 0.15 acre of wetlands (PEM) for the purpose of constructing Heron's Landing Golf Course. The total replacement area for the entire project will be 0.88 acre. To fill 0.083 acre of wetlands adjacent to the Dillersville Swamp to construct an entrance road from US Rt. 30.
