Availability of Multipathway Risk Assessment Report on the hazardous waste combustion activities at the Keystone Cement Company, Bath, PA.
The Department of Environmental Protection received the Multipathway Risk Assessment for The Keystone Cement Company Facility, Bath, PA report on Wednesday, July 9, 1997, along with the Plan Approval Applications to increase the hazardous waste feed rate from 50% to 75% of the total heat input, for each kiln.
On February 28, 1997, the Department and Keystone Cement Company entered into a Second Partial Consent Adjudication wherein Keystone agreed to submit the complete risk assessment report to the Department within 120 days of the effective date of the Second Partial Consent Adjudication. This risk assessment was to be performed in accordance with the Department approved Protocol and the Department's Risk Assessment Guidelines for Facilities Burning Hazardous Waste. The Department agreed to evaluate the risk assessment and any additional information submitted by Keystone within 120 days of receipt thereof. In addition, the Department's environmental consultant will be reviewing the report and providing the Department with comments. The Department also agreed to review the plan approval applications within the 120 day time period.

Proposed Revision to the State Implementation Plan for Oxides of Nitrogen (NOx) and Volatile Organic Compounds (VOC), and Notice of Public Hearing
Approval of Reasonably Available Control Technology (RACT) Plans for: J. G. Furniture Group, Inc. (Quakertown Borough, Bucks County), PECO Energy Company (West Conshohocken Borough, Montgomery County), Avery Dennison Corp./Fasson Roll Division (Richland Township, Bucks County), Sun Company, Inc. (Marcus Hook Borough, Delaware County)
DEP has made a preliminary determination to approve RACT plans as amendments to the State Implementation Plan (SIP) for the above facilities. The proposed SIP revisions do not adopt any new regulations. They incorporate the provisions and requirements contained in RACT approvals for these facilities to comply with current regulations.
These preliminary determinations, if finally approved, will be incorporated into Plan Approvals and/or Operating Permits for the facilities and will be submitted to the U. S. Environmental Protection Agency (EPA) as revisions to Pennsylvania's SIP.

One public hearing will be held for the purpose of receiving comments on the proposed SIP revisions. The hearing will represent the opportunity for oral comment to DEP on the proposed SIP revisions and will not be a question and answer session. Proposed Revision to the State Implementation Plan for Nitrogen Oxides (NOx); Approval of a Reasonably Available Control Technology Plan and Operating Permit; Public Hearing
Dauphin County The Department of Environmental Protection has made a preliminary determination to approve a Reasonably Available Control Technology (RACT) Plan and an amendment to the State Implementation Plan (SIP) for Hershey Chocolate USA's chocolate candy and confectionery manufacturing facility located in Derry Township, Dauphin County.
The proposed SIP revision does not adopt any new regulations. It incorporates the provisions and requirements contained in RACT Operating Permit No. 22-2004A for the existing Hershey East Plant to comply with current regulations. Upon final approval, the operating permit will include emission limits, fuel usage restrictions, recordkeeping and reporting requirements for four boilers. The permit will also establish emission reduction credits (ERC) available to the company from the previous shutdown of one boiler and the curtailment of two of the remaining boilers.
An appointment to review the pertinent documents at the Southcentral Regional Office may be scheduled by contacting Mary DiSanto at (717) 540-5018 between 8 a.m. and 3:30 p.m., Monday through Friday, except holidays.

E02-1135R. Encroachment. Borough of Franklin Park, 2428 Rochester Road, Sewickley, PA 15143-8608. To construct and maintain a 10-inch D.I.P. sewer line extension and a 10-inch D.I.P. storm pipe through two existing 60-inch CMP located under Interstate 79 (I-79), to place and maintain fill in de minimus area of wetlands less than or equal to 0.05 acre and to relocate and stabilize 155' of stream channel as part of the Fish Run Interceptor Sewer System Project. To place and maintain fill in approximately 0.1 acre of wetland (EM) for the purpose of adding additional commercial parcels (Phase III) to the Pointe of North Fayette development site. E02-1192. Encroachment. Millvale Borough, 501 Lincoln Avenue, Millvale, PA 15209. To remove the existing structure (North Avenue Bridge No. 4) and to construct and maintain a new structure having a span of 24.3 feet with an underclearance of 8.3 feet across Girtys Run (WWF). The structure is located on North Avenue, just north of the intersection of Klopper Street and North Avenue (Pittsburgh East, PA Quadrangle N: 20.0 inches; W: 13.6 inches) in Millvale Borough, Allegheny County. E11-249. Encroachment. East Carroll Township, P. O. Box 347, Carrolltown, PA 15722. To remove the existing structure and to construct and maintain a 25.7-foot long, 8-foot × 5-foot box culvert (invert depressed one foot) in an unnamed tributary to Laurel Lick Run (HQ-CWF) and to construct and maintain riprap for a total length of 50 LT at both approaches of the structure. The project includes construction and maintenance of two 36-inch pipe culverts as temporary road crossings. The project is located on T-468 (Farabaugh Rd) at a point approximately 0.05 mile north of intersection of T-468 and T-465 (Carrolltown, PA Quadrangle N: 13.1 inches; W: 6.3 inches) in East Carroll Township, Cambria County. To place and maintain fill in 3.63 acres of wetlands as part of the development of the Fayette Business Park Development Project located on TR 0510 approximately 0.5 mile east of its intersection with SR 0043 (Smithfield, PA Quadrangle N: 10.9 inches; W: 3.70 inches) in Georges Township, Fayette County. Permittee to provide 3.72 acres of replacement wetlands within the Fayette Business Park.

The park will consist of new pavilions, an amphitheater, new walkways, a gabion wall along the left bank of said stream, repairing and repaving an existing boat launch and roadway, dredging an area 1,000 feet upstream to downstream and approximately 600 feet from the left bank of said stream to the near side of the navigational channel to a depth no greater than 13.0 feet from the normal pool water surface elevation, and performing minor landscaping of the park. The project is located just upstream and downstream of the Kittanning Highway Bridge on the south side of South Water Street (Kittanning, PA Quadrangle N: 11.0 inches; W: 2.7 inches) in Kittanning Borough, Armstrong County.

To operate and maintain an 8-foot smooth metal pipe culvert authorized for construction under Emergency Permit No. EP5696220 in Gladdens Run. Said culvert is located on TR 357 approximately 1.1 miles northwest of its intersection with SR 2013 (Fairhope, PA Quadrangle N: 5.3 inches; W: 9.3 inches) in Southampton Township, Somerset County.
E56-276. Encroachment. John J. Cable, 126 Soap Hollow Rd, R. R. 2, Holsopple, PA 15935. To remove approximately 200 feet of existing 36-inch CMP stream enclosure from an unnamed tributary to South Fork of Bens Creek and to restore and stabilize the channel for the purpose of eliminating flooding and erosion problems. The project is located near the intersection of Soap Hollow Road and Moonlite Park Road (Hooversville, PA Quadrangle N: 18.6 inches; W: 13.1 inches) in Conemaugh Township, Somerset County.

To operate and maintain the existing 4-foot diameter culvert in an unnamed tributary to Brush Run (WWF) and the relocated channel of an unnamed tributary to Brush Run. This project was done under a compliance order and agreement and is located on the east side of Main Street, approximately 5,800 feet north from the intersection of Main Street and SR 980 (Canonsburg, PA Quadrangle N: 7.9 inches; W: 10.6 inches) in Cecil Township, Washington County. This permit was issued under section 105.13(e) ''small projects.'' This permit also includes 401 Water Quality Certification.

To remove the existing structure and to construct and maintain a bridge having a span of 25.0 feet with a minimum underclearance of 5.0 feet across an unnamed tributary to Mingo Creek (HQ-TSF) to provide access to applicant's property. The project is located on the west side of Little Mingo Creek approximately 400 feet west from the intersection of Beagle Club Road and Little Mingo Road (Hackett, PA Quadrangle N: 16.4 inches; W: 2.5 inches) in Nottingham Township, Washington County.

To remove the existing bridge and to construct and maintain a prestressed concrete box beam bridge having a normal span of 48.31 feet and an underclearance of 11.6 feet over Chartiers Creek (WWF). During construction, temporary stream diversions will be maintained in Chartiers Creek. The project is located along Country Club Road (SR 4049) approximately 600 feet south of its intersection with SR 4049 (Washington East, PA Quadrangle N: 15.7 inches; W: 16.0 inches) in Chartiers and South Strabane Townships, Washington County.
EA26-001SW. Environmental Assessment. West PA Coalition for Abandoned Mine Reclamation, Donohoe Ctr., R. R. 12, Box 202B, Greensburg, PA 15601. To periodically place high-quality alkaline (limestone) sand in the floodway of Glade Run (HQ-CWF) and two unnamed tributaries to Glade Run. This will allow high flow events to distribute the material downstream where it would dissolve to mitigate non-point source acid mine drainage by reducing acidity levels and raising the stream pH. These sites are located within State Game Lands 52. The site along Glade Run is located at its intersection with TR-722 (Fort Necessity, PA Quadrangle N: 21.8 inches; W: 12.2 inches) in Wharton Township, Fayette County.

To place fill in approximately 0.09 acre of wetlands on Lot 14 of the Durkee Farms Subdivision to prepare the lot for residential construction. The project is located approximately 1,000 feet east of the intersection of West End Road (S. R. 2005) and Lyndwood Avenue (Wilkes-Barre West, PA Quadrangle N: 19.8 inches; W: 6.5 inches), in Hanover Township, Luzerne County. The permittee has met the wetland replacement requirement by participating in the Pennsylvania Wetland Replacement Project.

To maintain a private bridge having a single span of approximately 18.0 feet and underclearance of 5.5 feet across a tributary to Appenzell Creek. The deteriorated substructure of this bridge was reconstructed under Emergency Permit No. EP4596411. The project is located approximately 0.2 mile west of the intersection of S. R. 3026 and S. R. 3021 (Saylorsburg, PA Quadrangle N: 19.5 inches; W: 15.0 inches) in Jackson Township, Monroe County.

To remove a timber deck single lane bridge on Henning Road, with a span of 10 feet, and an average underclearance of 4 feet, and to construct and maintain a low flow stream crossing consisting of a 24-foot long, 12-foot by 5-foot precast reinforced concrete box culvert in and along Deep Creek (TSF), impacting a de minimis amount of wetland (PSS). This site is located approximately 200 feet north of the junction of Henning Road and Deep Creek Road, (Sassamansville, PA Quadrangle N: 16.0 inches; W: 1.5 inches), in New Hanover Township, Montgomery County.

To construct and maintain a 45-foot long by 6-foot wide pedestrian/bike path bridge, which spans the Skippack Creek (TSF).
