SAFE DRINKING WATER


Permits issued for Construction Permit under the Pennsylvania Safe Drinking Water Act (35 P. S. §§ 721.1--721.17).

   Regional Office:  Northcentral Field Operations, Environmental Program Manager, 208 West Third Street, Suite 101, Williamsport, PA 17701.

   Permit No. Minor Amendment. The Department issued a construction permit to Geisinger Medical Center, 100 North Academy Avenue, Danville, PA 17822, Mahoning Township, Montour County. This permit authorizes the elimination of the existing 5,000 gallon inground concrete tank (wet sump) and related chlorine dioxide injection point and the rerouting of chlorinated Well No. 3 water directly to the existing 500,000 gallon storage tank.

Public Water Supply Permit 4189516 is being revised to withdraw the approval for use of Well No. 2 as a public water supply and to require the proper sealing and abandonment of the source upon completion of SWIP monitoring.

LAND RECYCLING AND ENVIRONMENTAL REMEDIATION

The following final reports were submitted under the Land Recycling and Environmental Remediation Standards Act (35 P. S. §§ 6026.101--6026.908).

   Provisions of Chapter 3 of the Land Recycling and Environmental Remediation Standards Act (act) require the Department of Environmental Protection (Department) to publish in the Pennsylvania Bulletin a notice of submission of final reports. A final report is submitted to document cleanup of a release of a regulated substance at a site to one of the act's remediation standards. A final report provides a description of the site investigation to characterize the nature and extent of contaminants in environmental media, the basis for selecting the environmental media of concern, documentation supporting the selection of residential or nonresidential exposure factors, a description of the remediation performed, and summaries of sampling methodology and analytical results which demonstrate that the remediation has attained the cleanup standard selected.

   For further information concerning the final report, contact the Environmental Cleanup Program Manager in the Department Regional Office under which the notice of receipt of a final report appears. If information concerning a final report is required in an alternative form, contact the community relations coordinator at the appropriate regional office listed.    Gregory Yocum Residence, Coal Township, Northumberland County. Marshall Miller and Associates, on behalf of its client Gregory Yocum, 1511 West Arch Street, Coal Township, PA 17866, has submitted a Final Report concerning the remediation of site soil contaminated with BTEX and PHCs. The report is intended to document remediation of the site to meet the Statewide health standard. See additional information in another section of this Pennsylvania Bulletin.

   Janet Gass Residence, City of Sunbury, Northumberland County. EarthRes Group, Inc. on behalf of their client, Janet Gass, 805 Market Street, Sunbury, PA 17801, have submitted a Final Report concerning the remediation of site soil contaminated with BTEX and PAHs. The Final Report is intended to document remediation of the site to meet the Statewide health standard.

   Herman Banks Scrap Yard, West Chillisquaque Township, Northumberland County. A Final Report was submitted on December 21, 1998 concerning the remediation of site soil contaminated with PCBs, heavy metals, PHCs and PAHs. The Final Report was submitted to document remediation of site-specific standards. The Department on January 20, 1999 approved the Final Report and the approval of the Final Report was published in the Pennsylvania Bulletin on February 20, 1999.

LAND RECYCLING AND ENVIRONMENTAL REMEDIATION

The Department has taken action on the following plans and reports under the Land Recycling and Environmental Remediation Standards Act (35 P. S. §§ 6026.101--6026.908) and Chapter 250 Administration of Land Recycling Program.

   Provisions of 25 Pa. Code § 250.8 Administration of Land Recycling Program requires the Department of Environmental Protection (Department) to publish in the Pennsylvania Bulletin a notice of its final actions on plans and reports. A final report is submitted to document cleanup of a release of a regulated substance at a site to one of the remediation standards of the Land Recycling and Environmental Remediation Standards Act (act). Plans and reports required by provisions of the act for compliance with selection of remediation to a site-specific standard, in addition to a final report, include a remedial investigation report, risk assessment report, and cleanup plan. A remedial investigation report includes conclusions from the site investigation, concentration of regulated substances in environmental media, benefits or reuse of the property, and in some circumstances, a fate and transport analysis. If required, a risk assessment report describes potential adverse effects caused by the presence of regulated substances. A cleanup plan evaluates the abilities of potential remedies to achieve remedy requirements. A final report provides a description of the site investigation to characterize the nature and extent of contaminants in environmental media, the basis for selecting the environmental media of concern, documentation supporting the selection of residential or nonresidential exposure factors, a description of the remediation performed, and summaries of sampling methodology and analytical results which demonstrate that the remediation has attained the cleanup standard selected. The Department may approve or disapprove plans and reports submitted. This notice provides the Department's decision and, if relevant, the basis for disapproval.

   Distribution Pole No. 40417S30397 (Site 135), Borough of Lititz, Lancaster County. PP&L, Inc., Two North Ninth Street, Allentown, PA 18101-1179 and Long and Bomberger, 45 North Broad Street, Lititz, PA 17543 have submitted a final report concerning the remediation of site soils contaminated with PCBs. The final report demonstrated attainment of the Statewide health standard, and was approved by the Department on July 28, 2000.

   Burle Industries, Inc., Lancaster City, Lancaster County. A Final Report has been submitted concerning the remediation of site groundwater contaminated with solvents. The Final Report demonstrated attainment of the site-specific standard, and was approved by the Department on November 9, 1998.

   Former Murata Electronics NA, Inc., CREDC Property, Carlisle Borough, Cumberland County. A Remedial Investigation Report and Final Report were submitted concerning remediation of site soils contaminated with solvents. The Remedial Investigation Report was approved September 4, 1998. The Final Report demonstrated attainment of the site-specific standard, and was approved by the Department on September 4, 1998.

   Assemco, Inc., Ridley Park Borough, Delaware County. A Final Report concerning remediation of site soil contaminated with petroleum hydrocarbons was submitted to the Department. The report demonstrated attainment of the Statewide health standard and was approved by the Department on November 12, 1998.

   One Montgomery Associates, Norristown Borough, Montgomery County. A Final Report concerning remediation of site soils contaminated with PCBs was submitted to the Department. The report demonstrated attainment of the Statewide health standard and was approved by the Department on October 6, 1998.

   Boeing Company, Ridley Township, Delaware County. A Final Report concerning remediation of site groundwater contaminated with solvents was submitted to the Department. The report demonstrated attainment of the Statewide health standard and was approved by the Department on October 9, 1998.

   Gilbert's Flower Shop Property (former), Ridley Township, Delaware County. A Final Report concerning remediation of site soils and groundwater contaminated with petroleum hydrocarbons was submitted to the Department. The report demonstrated attainment of the Statewide health standard and was approved by the Department on November 25, 1998.

   Northcentral Regional Office:  Michael C. Welch, Environmental Cleanup Program Manager, 208 West Third Street, Suite 101, Williamsport, PA 17701-6448, (570) 321-6525.

   Pennsylvania Power & Light Company (PPL)--Distribution pole #30780N37199, Muncy Creek Township, Lycoming County. A Final Report has been submitted addressing soil contaminated with PCBs (polychlorinated biphenyls). The report documented remediation to meet the Statewide health standard. The Department approved the report on October 8, 1998.

   USA Waste Management, West Chillisquaque Township, Northumberland County. Wade Fuchs, Waste Management, Inc., R. D. 2, Box 282A, Pleasant Valley Road, Irwin, PA 15642 has submitted a Final Report on October 26, 1998, showing attainment of the Statewide health standard for all constituents of diesel fuel resulting from a diesel spill at a truck accident involving USA Waste Management on August 10, 1998. The Final Report was approved by the Department on November 20, 1998.

   Ann Wilson Residence, City of Williamsport, Lycoming County. Response Environmental, Inc. on behalf of their client, Ann Wilson, 2011 Linwood Avenue, Williamsport, PA 17701, has submitted a Final Report concerning the remediation of site soil contaminated with BTEX and PAHs. The Final Report demonstrated attainment of the Statewide health standard and was approved by the Department on July 31, 2000.

   Southwest Field Office:  John J. Matviya, Environmental Cleanup Program Manager, 400 Waterfront Drive, Pittsburgh, PA 15222-4745, (412) 442-5217.

   ARCO Chemical Company, Beaver Valley Plant (Raccoon Creek), Potter Township, Beaver County. Thomas J. Walsh, ARCO Chemical Company, 3801 West Chester Pike, Newtown Square, PA 19073, has submitted a Remedial Investigation Report, Baseline Risk Assessment Report, and Cleanup Plan concerning remediation of site soils, groundwater and surface water contaminated with polycyclic aromatic hydrocarbons, petroleum hydrocarbons, benzene, toluene, ethylbenzene and xylene. The Department on October 9, 1998 approved the Remedial Investigation Report, Baseline Risk Assessment Report and Cleanup Plan.

   Tom Morris Residence, Perry Township, Fayette County. Kevin P. Van Kuren, Hydrocon Services, Inc., 2945 South Pike Avenue, Allentown, PA 18103, has submitted a Remedial Investigation Report, Baseline Risk Assessment Report, Cleanup Plan and Final Report concerning remediation of site soil contaminated with benzene, toluene, ethylbenzene, xylene and polycyclic aromatic hydrocarbons. The Remedial Investigation Report, Baseline Risk Assessment Report, Cleanup Plan and Final Report demonstrated attainment of the site-specific standard and was approved by the Department on October 22, 1998.

   Johnstown Corporation, Johnstown, Cambria County. Brian I. Fitzpatrick, P.G., QST Environmental, 5205 Militia Hill Road, Plymouth Meeting, PA has submitted a Final Report concerning remediation of site soil and groundwater contaminated with petroleum hydrocarbons. The Final Report demonstrated attainment of the Statewide health standard and was approved by the Department on October 14, 1998.

   Northwest Regional Office:  Craig Lobins, Environmental Cleanup Program Manager, 230 Chestnut Street, Meadville, PA 16335, (814) 332-6648.

   National Forge Company, Area of Concern (AOC-6), One Front Street, Irvin, PA 16329, Brokenstraw Township, County of Warren, and Paul Wojciak, P. E. of Environmental Management Associates, has submitted a Remedial Investigation Report and a Final Report concerning the remediation of groundwater. The site has been found to be contaminated with PAHs and Cutting Oil. The Final Report demonstrated attainment of the Statewide health and site specific standards and was approved by the Department on August 4, 2000.

Permits issued under the Solid Waste Management Act (35 P. S. §§ 6018.101--6018.1003), the Municipal Waste Planning, Recycling and Waste Reduction Act (53 P. S. §§ 4000.101--4000.1904) and regulations to operate solid waste processing or disposal area or site.

   Permit No. 600025. City of Philadelphia, Water Department, 1101 Market Street, Philadelphia, PA 19102. A permit modification was issued to modify the closure plan for the City's Philadelphia Northeast Airport sewage sludge trenching site. The modification calls for onsite sewage sludge and contaminated soils to be removed from unlined trenches and disposed in a new, lined landfill that will be constructed on the site. The new landfill and waste reburial project will consolidate the permitted area from approximately 120 acres to 36 acres. The remaining acreage will be commercially developed after the waste has been removed. The permit was modified by the Southeast Regional Office on August 2, 2000.

This modification will effectively increase the facility's potential Volatile Organic Compound (VOC) emissions by approximately 33 tons per year. Conditions in the permit will limit the VOC content of the coatings in accordance with 25 Pa. Code § 129.52 with yearly VOC emissions limited to less than 50 tons per year based on a 12-month rolling total. The Plan Approval and Operating Permit will contain additional recordkeeping and operating restrictions designed to keep the facility operating within all applicable air quality requirements.

   The Department of Environmental Protection (Department) has taken the following actions on previously received Dam Safety and Encroachment permit applications, requests for Environmental Assessment approval and requests for Water Quality Certification under section 401 of the Federal Water Pollution Control Act (33 U.S.C.A. § 1341(a)).

   Persons aggrieved by this action may appeal, under section 4 of the Environmental Hearing Board Act (35 P. S. § 7514) and 2 Pa.C.S. §§ 501--508 and 701--704 (relating to the Administrative Agency Law), to the Environmental Hearing Board, 400 Market Street, Second Floor, P. O. Box 8457, Harrisburg, PA 17105-8457, (717) 787-3483. TDD users may contact the Board through the Pennsylvania Relay Service, (800) 654-5984. Appeals must be filed with the Environmental Hearing Board within 30 days of receipt of written notice of this action unless the appropriate statute provides a different time period. To remove the existing structure and to construct and maintain a 13.08-foot by 8.3-foot aluminum structural arch culvert in the channel of an unnamed tributary to Conodoguinet Creek in order to improve the alignment of Creek Road located about 1,000 feet from its confluence with Conodoguinet Creek (Carlisle, PA Quadrangle N:  20.8 inches; W:  10.0 inches) in North Middleton Township, Cumberland County. This permit was issued under section 105.13(e) ''Small Projects.'' This permit also includes 401 Water Quality Certification.

   E21-311. Encroachment. Leonard Greiz, Shenandoah Mobile Company, P. O. Box 459, 124 South Main Street, Edinburg, VA 22824. To construct a 70 foot high galvanized single steel pool communications tower with a 6.0 foot base diameter enclosed by a 16 foot by 16 foot, 7 foot high, chain link fence in the floodway of the Yellow Breeches Creek located next to an existing 1 1/2 story building at Front and Market Streets (Steelton, PA Quadrangle N:  18.3 inches; W:  15.5 inches) in New Cumberland Borough, Cumberland County.    E22-412. Encroachment. Georgetown Commons Associates, 308 East King Street, Lancaster, PA 17602. To relocate 40 linear feet of a tributary to the Susquehanna River (WWF) and impact 1.1 acres of wetlands, 0.35 acre of direct impact and 0.75 acre of indirect impact, at a point upstream of Jefferson Drive for the purpose of enlarging a residential subdivision (Steelton, PA Quadrangle N:  15.4 inches; W:  3.1 inches) in Lower Swatara Township, Dauphin County. The permittee is required to provide a minimum of 1.1 acres of replacement wetlands. This permit also includes 401 Water Quality Certification.
