Service of Notice of Motor Carrier Applications

 The following temporary authority and/or permanent authority applications for the right to render service as a common carrier or contract carrier in this Commonwealth have been filed with the Pennsylvania Public Utility Commission. Formal protests and petitions to intervene must be filed in accordance with 52 Pa. Code (relating to public utilities). A protest shall indicate whether it applies to the temporary authority application, the permanent authority application, or both. Filings must be made with the Secretary, Pennsylvania Public Utility Commission, P. O. Box 3265, Harrisburg, PA 17105-3265, with a copy served on the applicant by May 4, 2015. Documents filed in support of the applications are available for inspection and copying at the Office of the Secretary between 8 a.m. and 4:30 p.m., Monday through Friday, and at the business address of the respective applicant.

Applications of the following for approval to begin operating as common carriers for transportation of persons as described under each application.

A-2015-2471344. Whitemarsh Community Ambulance Association (P. O. Box 376, Lafayette Hill, Montgomery County, PA 19444) for the right to transport, as a common carrier, by motor vehicle, persons in paratransit service, from points in the Counties of Delaware, Montgomery and Philadelphia, to points in Pennsylvania, and return. A-2015-2472606. Cathay Xpress, Inc. (919 Wolf Street, Philadelphia, PA 19148) for the right to begin to transport, as a common carrier, by motor vehicle, persons in limousine service, between points in the Counties of Montgomery and Monroe, excluding service that is under the jurisdiction of the Philadelphia Parking Authority.

KDDS Services, LLC (2576 East Valley Road, Loganton, Clinton County, PA 17747) for the right to transport as a common carrier, by motor vehicle, persons in group and party service, in vehicles seating 11 to 15 passengers, including the driver, from points in the Counties of Centre, Clinton, Lycoming and Union, to points in Pennsylvania, and return, excluding service that is under the jurisdiction of the Philadelphia Parking Authority. Laye Kourouma t/a Kourouma Transport (203 West Baltimore Avenue, Lansdowne, Delaware County, PA 19050) for the right to transport, as a common carrier, by motor vehicle, persons in paratransit service, from points in the City and County of Philadelphia and the Counties of Bucks, Chester, Delaware and Montgomery, to points in Pennsylvania, and return.

Valley Forge Limo Co., LLC t/a VF Chauffeured (601 South Henderson Road, Suite 210, King of Prussia, Montgomery County, PA 19406) for the right to transport as a common carrier, by motor vehicle, persons in group and party service, in vehicles seating 11 to 15 passengers, including the driver, from points in the Counties of Berks, Bucks, Chester, Delaware, Lancaster, Lehigh, Luzerne and Montgomery, to points in Pennsylvania, and return, excluding service that is under the jurisdiction of the Philadelphia Parking Authority.

A-2015-2474010. Premier Limousine, LLC (106 Anderson Road, Curwensville, Clearfield County, PA 16833) for the right to transport as a common carrier, by motor vehicle, persons in group and party service, in vehicles seating 11 to 15 passengers, including the driver, from points in Pennsylvania, to points in Pennsylvania, and return, excluding service that is under the jurisdiction of the Philadelphia Parking Authority, the Pittsburgh Airport and the City of Philadelphia.

A-2015-2474729. Gary L. Snyder and Marie Snyder (2748 Schwaben Creek Road, Rebuck, PA 17867) persons whose personal convictions prevent them from owning and/or operating motor vehicles, in paratransit service, from points in Northumberland County, to points in Pennsylvania, and return.

Application of the following for approval of the additional right and privilege of operating motor vehicles as common carriers for transportation of persons as described under the application.

A-2015-2472707. Susquehanna Valley Limousine, Inc. t/a Susquehanna Valley Taxi Service (2225 Ridge Road, Northumberland, PA 17857) for the additional right, to transport by motor vehicle, persons, upon call or demand in the Counties of Northumberland, Montour, Union and Snyder.

Application of the following for approval of the beginning of the exercise of the right and privilege of operating motor vehicles as common carriers for the transportation of household goods as described under the application.

A-2015-2472602. Service Emporium, Inc. t/a Man With A Van (1328 Limekiln Pike, Dresher, Montgomery County, PA 19025) for the right to begin to transport, as a common carrier, by motor vehicle, household goods in use, between points in Pennsylvania.

Applications of the following for approval to begin operating as contract carriers for transportation of persons as described under each application.

A-2014-2456367. EZ Ride, LLC (P. O. Box 8083, Gaithersburg, MD 20898) a corporation of the Commonwealth of Pennsylvania—persons for Access2Care, LLC, under its Medical Assistance Transportation Program contract, between points in the Counties of Bucks, Chester, Delaware and Philadelphia.

A-2015-2465709. R M Home Services d/b/a Community Home Services (672 Main Street, Harleysville, PA 19438) for the right to begin to transport, as a contract carrier, by motor vehicle, persons from other nonprofit organizations, from points in the Townships of Milford, Richland, Springfield, Haycock, East Rockhill, West Rockhill, Bedminster, Hilltown, Plumstead, New Britain and Doylestown, all in Bucks County; and the Townships of Upper Hanover, Marlborough, Upper Frederick, Lower Frederick, Salford, Upper Salford, Lower Salford, Skippack, Worcester, East Norriton, Franconia, Montgomery, Hatfield, Upper Gwynedd, Lower Gwynedd and Towa-mencin, all in Montgomery County, to points in Pennsylvania and return.
