Service of Notice of Motor Carrier Applications

 The following temporary authority and/or permanent authority applications for the right to render service as a common carrier or contract carrier in this Commonwealth have been filed with the Pennsylvania Public Utility Commission. Formal protests and petitions to intervene must be filed in accordance with 52 Pa. Code (relating to public utilities). A protest shall indicate whether it applies to the temporary authority application, the permanent authority application, or both. Protests may only be filed if there is evidence that the applicant lacks fitness. Protests based on endangering or impairing operations of an existing carrier will not be honored. Filings must be made with the Secretary, Pennsylvania Public Utility Commission, 400 North Street, Harrisburg, PA 17120, with a copy served on the applicant by June 17, 2019. Documents filed in support of the applications are available for inspection and copying at the Office of the Secretary between 8 a.m. and 4:30 p.m., Monday through Friday, and at the business address of the respective applicant.

Applications of the following for approval to begin operating as common carriers for transportation of persons as described under each application.

A-2018-3004732. Angela's County Taxi, LLC (2400 Darlington Road, Beaver Falls, PA 15010) for the right to begin to transport, by motor vehicle, persons, upon call or demand, in the Counties of Beaver, Lawrence and Mercer. Attorney: David W. Donley, 3361 Stafford Street, Pittsburgh, PA 15204-1441.

A-2019-3007051. Rockville Rides, LLC (300 Treaster Road, Belleville, PA 17004) for the right to begin to transport, as a common carrier, persons in group and party service, in vehicles seating 11 to 15 passengers, including the driver, from points in the County of Mifflin, to points in Pennsylvania, and return.

A-2019-3008556. Dolores Jiménez De La Rosa, t/a De La Rosa's Transportation (823 West Front Street, Berwick, Columbia County, PA 18603) persons in paratransit service, within a 100-mile radius of the city limits of Butler, Butler County.

A-2019-3009800. Shadai Healthcare Services, LLC, t/a Shadai Home Health Services, LLC (3507 Market Street, Camp Hill, Cumberland County, PA 17011) for the right to begin to transport, as a common carrier, by motor vehicle, persons in paratransit service, between points in the Counties of Adams, Berks, Centre, Cumberland, Dauphin, Franklin, Fulton, Huntingdon, Juniata, Lancaster, Lebanon, Lehigh, Montgomery, Perry, Union and York, and the City and County of Philadelphia.

A-2019-3009882. Mlaker, LLC (1976 Valley View Drive, Davidsville, PA 15928) for the right to begin to transport persons, by motor vehicle, in paratransit service, between points in the Counties of Cambria and Somerset.

Application of the following for the approval of the right and privilege to discontinue/abandon operating as common carriers by motor vehicle and for cancellation of the certificate of public convenience as described under the application.

A-2019-3009981. Jerome Miller (154 Johnson Road, Delta, York County, PA 17314) for the discontinuance of service and cancellation of its certificate, at A-00122786, as a common carrier, by motor vehicle, authorizing the transportation of persons in paratransit service, limited to those whose personal convictions prevent them from owning or operating motor vehicles, from points in the Counties of Lancaster, York and Chester, to points in Pennsylvania, and return.
