Automatic Suspension of the License to Practice Practical Nursing of Melinda S. Paul, LPN; Case No. 18-51-008743

Notice to Melinda S. Paul, LPN:

 The Commonwealth of Pennsylvania, Department of State, Bureau of Professional and Occupational Affairs, instituted formal administrative action against you by filing a Petition for Automatic Suspension before the State Board of Nursing (Board) alleging that your license to practice practical nursing is subject to automatic suspension under section 23(c) of the Controlled Substance, Drug, Device and Cosmetic Act (''Drug Act''). This notice is being published pursuant to 1 Pa. Code § 33.31 and Rule 430 of the Pennsylvania Rules of Civil Procedure, 231 Pa. Code Rule 430, providing service of process upon you by publication.

 Your license to practice practical nursing in the Commonwealth of Pennsylvania was AUTOMATICALLY SUSPENDED under the authority of Section 23(c) of the Drug Act, EFFECTIVE MAY 31, 2019.

 If you wish to defend against the charges in the Petition or request a hearing you must do so within twenty days from the date of publication of this Notice. The procedure for doing so is explained in the Petition, Notice and Order of Automatic Suspension. You may obtain a copy of the Petition, Notice and Order online at www.pals.pa.gov or by contacting the Prothonotary for the Department of State, 2601 North Third Street, P.O. Box 2649, Harrisburg, PA 17105-2649, (717) 772-2686.

 You have the right to retain an attorney. Although you may represent yourself without an attorney, you are advised to seek the assistance of an attorney. All proceedings before the Board are conducted under the Administrative Agency Law and the General Rules of Administrative Practice and Procedure. If a response and request for a hearing are not filed within 20 days of publication of this Notice, an order making the suspension of your license final will be issued by the State Board of Nursing. The response and any pleadings or other documents related to this matter must be filed with the Prothonotary for the Department of State at the above address. A copy of the response and request for hearing shall also be served on the prosecuting attorney identified in the Petition.
