 On September 19, 2018, David Sabol, t/d/b/a David Sabol's Barber Shop, license No. BO433269, of Ambridge, Beaver County, was levied a civil penalty of $500 for operating on a lapsed or expired license.

 Individuals may obtain a copy of the final order by writing to Ronald K. Rouse, Board Counsel, State Board of Barber Examiners, P.O. Box 69523, Harrisburg, PA 17106-9523.

 This order represents the final State Board of Barber Examiners (Board) decision in this matter. It may be appealed to the Commonwealth Court of Pennsylvania by the filing of a petition for review with that court in accordance with the Pennsylvania Rules of Appellate Procedure. Individuals who take an appeal to the Commonwealth Court must serve the Board with a copy of their petition for review. The Board contact for receiving service of appeals is the previously-named Board counsel.
