PROPOSED NUTRIENT MANAGEMENT PLANS RELATED TO APPLICATIONS FOR NPDES PERMITS FOR CAFOs

 This notice provides information about agricultural operations that have submitted nutrient management plans (NMPs) for approval under 3 Pa.C.S. Chapter 5 and that have or anticipate submitting applications for new, amended or renewed (National Pollutant Discharge Elimination System) NPDES permits, or Notices of Intent (NOIs) for coverage under a general permit, for CAFOs, under 25 Pa. Code Chapter 92a. This notice is provided in accordance with 25 Pa. Code Chapter 92a and 40 CFR Part 122, implementing The Clean Streams Law and the Federal Clean Water Act.

 Based upon preliminary reviews, the State Conservation Commission (SCC) or County Conservation Districts (CCD) working under a delegation agreement with the SCC have completed an administrative review of NMPs described. These NMPs are published as proposed plans for comment prior to taking final actions. The NMPs are available for review at the CCD office for the county where the agricultural operation is located. A list of CCD office locations is available at http://www.nacdnet.org/about/districts/directory/pa.phtml or can be obtained from the SCC at the office address listed or by calling (717) 787-8821.

 Persons wishing to comment on an NMP are invited to submit a statement outlining their comments on the plan to the CCD, with a copy to the SCC for each NMP, within 30-days from the date of this public notice. Comments received within the respective comment periods will be considered in the final determinations regarding the NMPs. Comments should include the name, address and telephone number of the writer and a concise statement to inform the SCC of the exact basis of the comments and the relevant facts upon which they are based.  Under the Pennsylvania Safe Drinking Water Act (35 P.S. §§ 721.1—721.17), the following parties have applied for PWS permits to construct or substantially modify public water systems.

 Persons wishing to comment on permit applications are invited to submit statements to the office listed before the application within 30 days of this public notice. Comments received within this 30-day comment period will be considered in the formulation of the final determinations regarding an application. A comment should include the name, address and telephone number of the writer and a concise statement to inform the Department of the exact basis of a comment and the relevant facts upon which it is based. A public hearing may be held after consideration of comments received during the 30-day public comment period.

 Following the comment period, the Department will make a final determination regarding the proposed permit. Notice of this final determination will be published in the Pennsylvania Bulletin at which time this determination may be appealed to the Environmental Hearing Board.

LAND RECYCLING AND ENVIRONMENTAL REMEDIATION

UNDER ACT 2, 1995
PREAMBLE 1


Acknowledgment of Notices of Intent to Remediate Submitted Under the Land Recycling and Environmental Remediation Standards Act (35 P.S. §§ 6026.101—6026.908).

 Sections 302—305 of the Land Recycling and Environmental Remediation Standards Act (Act) require the Department of Environmental Protection (Department) to publish in the Pennsylvania Bulletin an acknowledgment noting receipt of any Notices of Intent to Remediate. An acknowledgment of the receipt of a Notice of Intent (NOI) to Remediate is used to identify a site where a person proposes to, or has been required to, respond to a release of a regulated substance at a site. Persons intending to use the background standard, Statewide health standard, the site-specific standard, or who intend to remediate a site as a special industrial area, must file a (NOI) to Remediate with the Department. A NOI to Remediate filed with the Department provides a brief description of the location of the site, a list of known or suspected contaminants at the site, the proposed remediation measures for the site, and a description of the intended future use of the site. A person who demonstrates attainment of one, or a combination of the cleanup standards, or who receives approval of a special industrial area remediation identified under the Act, will be relieved of further liability for the remediation of the site for any contamination identified in reports submitted to and approved by the Department. Furthermore, the person shall not be subject to citizen suits or other contribution actions brought by responsible persons not participating in the remediation.

 Under Sections 304(n)(1)(ii) and 305(c)(2) of the Act, there is a 30-day public and municipal comment period for sites proposed for remediation using a site-specific standard, in whole or in part, and for sites remediated as a special industrial area. This period begins when a summary of the NOI to Remediate is published in a newspaper of general circulation in the area of the site. For the following identified site(s), proposed for remediation to a site-specific standard or as a special industrial area, the municipality, within which the site is located, may request to be involved in the development of the remediation and reuse plans for the site if the request is made within 30-days of the following specified date. During this comment period the municipality may request that the following identified person, as the remediator of the site, develop and implement a public involvement plan. Requests to be involved, and comments, should be directed to the remediator of the site.

 For further information concerning the content of a NOI to Remediate, please contact the Environmental Cleanup Program Manager in the Department of Environmental Protection Regional Office under which the notice appears. OPERATE WASTE PROCESSING OR DISPOSAL AREA OR SITE

AIR QUALITY

PLAN APPROVAL AND OPERATING PERMIT APPLICATIONS

 The Department has developed an ''integrated'' plan approval, State Operating Permit and Title V Operating Permit program. This integrated approach is designed to make the permitting process more efficient for the Department, the regulated community and the general public. This approach allows the owner or operator of a facility to submit permitting documents relevant to its application for all sources related to a facility or a proposed project, affords an opportunity for public input, and provides for a decision on the issuance of the necessary permits.

 The Department received applications for Plan Approvals or Operating Permits from the following facilities. Copies of the application, the Department's analysis, all pertinent documents used in the evaluation of the application and subsequently prepared proposed plan approvals/operating permits are available for public review during normal business hours at the appropriate Department Regional Office. Appointments for scheduling a review must be made by calling the appropriate Department Regional Office. The address and phone number of the Regional Office is listed before the application notices.

 Persons wishing to file a written protest or provide comments or additional information, which they believe should be considered prior to the issuance of a permit, may submit the information to the Department's Regional Office. A 30-day comment period from the date of this publication will exist for the submission of comments, protests and information. Each submission must contain the name, address and telephone number of the person submitting the comments, identification of the proposed Plan Approval/Operating Permit including the permit number and a concise statement regarding the relevancy of the information or objections to issuance of the permit.

 A person wishing to request a hearing may do so during the 30-day comment period.  Permits issued to the owners or operators of sources subject to 25 Pa. Code Chapter 127, Subchapter D or E, or located within a Title V facility or subject to 25 Pa. Code § 129.51(a) or permits issued for sources with limitations on their potential to emit used to avoid otherwise applicable Federal requirements may be submitted to the United States Environmental Protection Agency for review and approval as a revision to the State Implementation Plan. Final Plan Approvals and Operating Permits will contain terms and conditions to ensure that the sources are constructed and operating in compliance with applicable requirements in the Air Pollution Control Act (35 P.S. §§ 4001—4015), 25 Pa. Code Chapters 121—145, the Federal Clean Air Act (42 U.S.C.A. §§ 7401—7671q) and regulations adopted under the Federal Clean Air Act.

67-03187A: Penn Mar Recycling Co., Inc. (14 Onion Blvd., Shrewsbury, PA 17361) for the installation of a United Group AS-1000 aluminum sweat furnace at the facility located in Shrewsbury Borough, York County. The sweat furnace is equipped an integral afterburner. The expected potential emissions from this project are as follows: 1.35 tpy CO; 2.33 tpy NOx; and less than 1 tpy of PM-10, SOx, VOC, and HAPs. The Department of Environmental Protection's (DEP's) review of the information submitted by the company indicates that the air contamination sources will comply with all regulatory requirements, including monitoring, recordkeeping, and reporting requirements, and pertaining to air contamination sources and the emission of air contaminants including the best available technology requirement (BAT) of 25 Pa. Code §§ 127.1 and 127.12. The facility is also subject to 40 CFR 63 Subpart RRR—National Emission Standards for Hazardous Air Pollutants for Secondary Aluminum Production. DEP proposes to issue a plan approval for the proposed installation. If DEP determines that the sources are constructed and operated in compliance with the plan approval conditions and the specifications of the application for plan approval, the applicant will subsequently apply for an Operating Permit in accordance with 25 Pa. Code Chapter 127, Subchapter F.

OPERATING PERMITS


Intent to Issue Title V Operating Permits under the Air Pollution Control Act (35 P.S. §§ 4001—4015) and 25 Pa. Code Chapter 127, Subchapter G.

TVOP-65-00053: Dura Bond (P.O. Box 518, 2658 Puckety Drive, Export, PA 15632). In accordance with 25 Pa. Code §§ 127.424, 127.425 and 127.521, the Department of Environmental Protection (DEP) is providing notice that it intends to issue a Title V Operating Permit renewal to Dura-Bond Steel Corporation for their facility located in Export Borough, Westmoreland County.

 Dura Bond is a specialty fabricator and coater of steel pipe and metal products. The primary operations at this facility include shot blasting, sand blasting, coating, and fabrication. The steel pipes and other metal products are cleaned by sandblasting or shot blasting. After cleaning, the materials are coated in the spray area using airless spray guns. Materials are then placed in the day down area for drying and curing. Sometimes, they are forced cured using portable natural gas heaters. Fabrication work is done in a separate building. It includes typical machine shop/metal fabrication equipment such as grinders, welding stations, saws, torching, etc. There are several baghouses to control particulate emissions.

 The pollutants of concern from this facility are VOCs and HAPs from the coating and cleaning operations. The overall facility's potential VOC emission is limited to 92 tons per 12 consecutive month period. The facility's maximum HAPs and PM10 emissions estimated to be respectively 45.66 tons per year and 23.62 tons per year. Other insignificant emissions are NOx, SOx, and CO from the use of natural gas fired heaters. Dura Bond is major for HAPs and subject to 40 CFR 63, Subpart MMMM. The proposed TVOP renewal contains emission restriction, testing, monitoring, recordkeeping, reporting and work practice standards derived from the applicable requirements of 25 Pa. Code Article III, Chapters 121—145 and 40 CFR 63, Subpart MMMM.

 Copies of the application, DEP's analyses and other documents used in the evaluation of the application are available for public review during normal business hours at the Department's Southwest Regional Office, 400 Waterfront Drive, Pittsburgh, PA 15222. Appointments for scheduling a review must be made by calling 412-442-4000.

 Any person may submit comments, a request for the Department to hold a public hearing, or a protest to the operating permit or a proposed condition thereof, by filing such submissions in writing with the Department at the Southwest Regional Office. A 30-day comment period from the date of this publication will exist for the submission of comments.

 All requests for a public hearing, and all protests to a proposed action, shall be filed with the Department within 30 days of the date that notice of the proposed action was published under 25 Pa. Code § 127.521(b) (relating to public notice). A protest must include the name, address and telephone number of the person filing the protest, identification of the proposed permit issuance being opposed (Operating Permit TV-65-00053) and a concise statement of the objections to the permit issuance and the relevant facts upon which the objections are based.

 A public hearing may be held in accordance with 25 Pa. Code § 127.429, if the Department, in its discretion, decides that such a hearing is warranted based on the information received. If a public hearing is held, all persons who have properly filed a protest under 25 Pa. Code § 127.426 may appear and give testimony. The applicant, the protestant and other participants will be notified of the decision to hold a hearing (and the time, place and purpose of such hearing) by publication in the newspaper or by the Pennsylvania Bulletin, or by telephone, where the Department determines such notification by telephone is sufficient.

09-00184: Doylestown Hospital (595 West State Street, Doylestown, PA 18901), for renewal of a State Only Operating Permit in Doylestown Township, Bucks County. The facility operates four (4) dual-fired boilers, one (1) natural gas 1.6 MW cogeneration unit, one (1) natural gas fired electric generator for peak shaving purposes, four (4) emergency diesel electric generators, and two (2) emergency diesel fire pumps. Doylestown Hospital is categorized as a synthetic minor facility for NOx emissions with a site-wide limit of 24.9 tons per year. There have been no changes to any of the existing sources and there are no new sources being added under this renewal and the permittee shall continue to comply with all existing requirements. The permit includes monitoring, recordkeeping and reporting requirements designed to address all applicable air quality requirements.

15-00150: The Vanguard Group, Inc. (1041 West Valley Rd, Valley Forge, PA) located in Tredyffrin Township, Chester County. This action is to renew a State-Only Operating Permit. The permit is for a total of seven (7) existing, previously exempted diesel-fired emergency generator sets. The permit will include monitoring, recordkeeping and reporting requirements designed to keep the facility operating within all applicable air quality requirements.

46-00127: Verizon Pennsylvania, LLC (1050 Virginia Drive, Fort Washington, PA 19034) located in Upper Dublin Township, Montgomery County for the renewal of their Air Quality State Only Operating Permit. This facility is located in an office building and provides telephone dial tone to the surrounding areas. Verizon owns and operates two emergency generators that are used to supply power to the facility in emergency situations when electric power from their utility provider is interrupted. Verizon has previously been classified as a synthetic minor facility for NOx emissions.

 Verizon has requested to be reclassified as a natural minor facility based on EPA memorandum ''Calculating Potential to Emit (PTE) for Emergency Generators'' dated September 6, 1995. EPA recommends that the potential to emit (PTE) be determined based upon an estimate of the maximum number of hours the generator could expect to operate for emergency situations only when the electric power supply is interrupted. EPA believes that 500 hours is an appropriate default assumption which is the default that Verizon is requesting to use to be reclassified as a natural minor. The EPA memorandum addresses only emergency generators with the generator engines being the only emitting sources at a facility.

 The operating permit limits each engine to an annual limit of 500 hours per year. With this limitation the facilities PTE for NOx is 17.80 tons per year which is below the major facility threshold of 25.00 tons per year. The Department has reclassified Verizon's Fort Washington Facility from a synthetic minor to a natural minor. The operating permit also contains monitoring, recordkeeping and work practice standard conditions designed to keep the facility operating within all applicable air quality requirements.

 Somerset Area School District operates four boilers, operable on coal or natural gas, and two natural gas-fired emergency generators. The facility is limited to 2,150 tons per year of coal usage. Coal sulfur content is limited to 2.3% and ash content to 10%. Each emergency generator is limited to 500 hours of operation per 12-consecutive month period. MINING ACTIVITY NPDES DRAFT PERMITS

 This notice provides information about applications for a new, amended or renewed NPDES permits associated with mining activity (coal or noncoal) permits. The applications concern industrial waste (mining) discharges to surface water and discharges of stormwater associated with mining activities.  A settleable solids instantaneous maximum limit of 0.5 ml/l applies to: surface runoff (resulting from a precipitation event of less than or equal to a 10-year 24-hour event) from active mining areas; active areas disturbed by coal refuse disposal activities; mined areas backfilled and revegetated; and all other discharges and drainage (resulting from a precipitation event of greater than 1-year 24-hour to less than or equal to a 10-year 24-hour event) from coal refuse disposal piles. Similarly, modified BAT limits apply to iron, manganese and suspended solids in surface runoff, discharges and drainage resulting from these precipitation events and those of greater magnitude in accordance with 25 Pa. Code §§ 87.102, 88.92, 88.187, 88.292, 89.52 and 90.102.

 Exceptions to BAT effluent limits may be applicable in accordance with 25 Pa. Code §§ 87.102, 88.92, 88.187, 88.292, 89.52 and 90.102.

Effluent Limits for Noncoal Mining Activities

 The limits for noncoal mining activities as provided in 25 Pa. Code Chapter 77 are pH 6 to 9 and other parameters the Department may require.

 Discharges from noncoal mines located in some geologic settings (for example, in the coal fields) may require additional water quality based effluent limits. If additional effluent limits are needed for an NPDES permit associated with a noncoal mining permit, then the permit description specifies the parameters.

 In addition to BAT or WQBEL limits, coal and noncoal NPDES permits establish effluent limitations in the form of implemented Best Management Practices (BMPs) identified in the associated Erosion and Sedimentation Plan, the Reclamation Plan and the NPDES permit application. These BMPs restrict the rates and quantities of associated pollutants from being discharged into surface waters in this Commonwealth.

 More restrictive effluent limitations, restrictions on discharge volume or restrictions on the extent of mining that may occur are incorporated into an NPDES permit when necessary for compliance with water quality standards and antidegradation requirements (in accordance with 25 Pa. Code Chapters 91—96).

 The procedures for determining the final effluent limits, using a mass-balance equation or model, are found in Technical Guidance Document 563-2112-115, Developing National Pollutant Discharge Elimination System (NPDES) Permits for Mining Activities. Other specific factors to be considered include public comments and Total Maximum Daily Load(s). Additional discharge limitations may apply in the event that unexpected discharges occur.

 Discharge rates for surface mining activities are precipitation driven. Discharge rates for proposed discharges associated with underground mining are noted in the permit description.

 Persons wishing to comment on an NPDES draft permit should submit a written statement to the Department at the address of the district mining office indicated before each draft permit within 30-days of this public notice. Comments received within the comment period will be considered in the final determinations regarding the NPDES permit applications. Comments must include the name, address and telephone number of the writer and a concise statement to inform the Department of the exact basis of a comment and the relevant facts upon which it is based.

 The Department will also accept requests or petitions for a public hearing on NPDES permit applications, as provided in 25 Pa. Code § 92a.82(d). The request or petition for a public hearing shall be filed within 30-days of this public notice and contain the name, address, telephone number and the interest of the party filing the request and state the reasons why a hearing is warranted. A public hearing may be held if the Department considers the public interest significant. If a hearing is scheduled, a notice of the hearing on the NPDES permit application will be published in the Pennsylvania Bulletin and a newspaper of general circulation within the relevant geographical area. When a public hearing is held, the Department will consider comments from the public hearing in the final determination on the NPDES permit application.

FEDERAL WATER POLLUTION CONTROL ACT SECTION 401

 The following permit applications, requests for Environmental Assessment approval and requests for 401 Water Quality Certification have been received by the Department of Environmental Protection. Section 401 of the Federal Water Pollution Control Act (FWPCA) (33 U.S.C.A. § 1341), requires the State to certify that the involved projects will not violate the applicable provisions of Sections 301—303, 306 and 307 of the FWPCA (33 U.S.C.A. §§ 1311—1313, 1316 and 1317), as well as relevant State requirements. Persons objecting to approval of a request for certification under Section 401 or to the issuance of a Dam Permit or Water Obstruction and Encroachment Permit, or the approval of an Environmental Assessment must submit any comments, suggestions or objections within 30-days of the date of this notice, as well as any questions to the office noted above the application. Comments should contain the name, address and telephone number of the person commenting, identification of the certification request to which the comments or objections are addressed, and a concise statement of comments, objections or suggestions including the relevant facts upon which they are based.

 The Department may conduct a fact-finding hearing or an informal conference in response to comments if deemed necessary.  PennDOT is proposing to perform the following water obstruction and encroachment activities for the reconstruction of Park Avenue (S.R. 4004) and Eagleville Road (S.R. 4006) associated with the overall Crawford Road (T-324), Eagleville Road (S.R. 4006), and Park Avenue (S.R. 4004) Realignment Project:

 1. To remove a 28-inch by 40-inch CMP pipe culvert on Park Avenue (S.R. 4004) carrying an unnamed tributary to Mine Run (TSF-MF, S3), and in its place, construct and maintain a 46-foot long, 48-inch by 76-inch HERCP culvert resulting in 46 linear feet (430 square feet, 0.010 acre) of permanent watercourse impact and 140 linear feet (1,526 square feet, 0.035 acre) of temporary watercourse impact. In addition, 13,452 square feet (0.309 acre) of permanent floodway impact and 7,194 square feet (0.165 acre) of temporary floodway impact as result of construction activities associated with roadway reconstruction, proposed grading, and access.

 2. To construct and maintain an 18-inch RCP outfall within the floodway of an unnamed tributary to Mine Run (TSF-MF, S2) resulting in 30 square feet (0.001 acre) of permanent floodway impact and is associated with grading and construction of a stormwater facility. This also includes the placement of riprap rock apron.

 3. To remove a 29-foot wide, 16-foot long, single-span reinforced concrete slab bridge on Park Avenue (S.R. 4004) over Mine Run (TSF-MF, S1), and in its place, construct and maintain a 61-foot long, 18-foot by 11-foot precast concrete box culvert resulting in 152 linear feet (2,851 square feet, 0.065 acre) of permanent watercourse impact and 74 linear feet (1,419 square feet, 0.033 acre) of temporary watercourse impact. In addition, there will be a total of 16,077 square feet (0.369 acre) of permanent floodway impact, 5,927 square feet (0.136 acre) of temporary floodway impact, 12,739 square feet (0.292 acre) of permanent floodplain impact, and 3,656 square feet (0.084 acre) of temporary floodplain impact due to roadway reconstruction/alignment changes, construction of stormwater facilities, grading, and access.

 4. To construct and maintain an 18-inch RCP outfall within the floodway of Mine Run (TSF-MF, S1) resulting in 30 square feet (0.001 acre) of permanent floodway impact and is associated with grading and construction of a stormwater facility. This also includes the placement of riprap rock apron.

 A water obstruction and encroachment permit (PADEP Permit No. E4601221-013) is also associated with the overall reconstruction/realignment of the intersection for the township-owned culvert (T-324 Crawford Road) and stream realignment. This project is located in Lower Providence Township, Montgomery County (USGS PA Collegeville Quadrangle—Latitude: 40.13376 N, Longitude: 75.42597 W).

Permit No. E4601221-013, Lower Providence Township, 100 Parklane Drive, Eagleville, PA 19403, Lower Providence Township, Montgomery County, ACOE Philadelphia District.

 Lower Providence Township is proposing to perform the following water obstruction and encroachment activities for the reconstruction of Crawford Road (T-324) associated with the overall Crawford Road (T-324), Eagleville Road (S.R. 4006), and Park Avenue (S.R. 4004) Realignment Project:

 1. To construct and maintain a new 64-foot long, 18-foot by 11-foot precast concrete box culvert carrying Mine Run (TSF-MF, S1) resulting in 58 linear feet (754 square feet, 0.018 acre) of permanent watercourse impact due to the placement of the culvert. In addition, there will be 31,268 square feet (0.719 acre) of permanent floodway impact, 9,021 square feet (0.208 acre) of temporary floodway impact, 18,983 square feet (0.436 acre) of permanent floodplain impact, and 6,879 square feet (0.185 acre) of temporary floodplain impact due to roadway reconstruction/realignment, construction of stormwater facilities, grading, and access. The construction and maintenance of a retaining wall is also included with this activity is located immediately upstream of the culvert left wingwall.

 2. To relocate and realign Mine Run (TSF-MF, S1) due to the placement of the previously stated culvert and realignment of Crawford Road (T-324) resulting in 272 linear feet (5,052 square feet, 0.116 acre) of permanent watercourse impact and 20 linear feet (523 square feet, 0.012 acre) of temporary watercourse impact.

 3. To temporarily impact an unnamed tributary to Mine Run (TSF-MF, S2) in order to construct the new Crawford Road culvert resulting in 40 linear feet (247 square feet, 0.006 acre) of temporary watercourse impact and 1,169 square feet (0.027 acre) of temporary floodway impact due to access and E&S BMPs.

 4. To construct and maintain an 18-inch RCP outfall within the floodway of Mine Run (TSF-MF, S1) resulting in 30 square feet (0.001 acre) of permanent floodway impact and is associated with grading and construction of a stormwater facility. This also includes the placement of riprap rock apron.

 5. To relocate, construct, and maintain a sewer line relocation resulting in 41 linear feet (871 square feet, 0.020 acre) of permanent watercourse impact and 2,744 square feet (0.063 acre) of temporary floodway impact.

 A water obstruction and encroachment permit (PADEP Permit No. E4601121-012) is also associated with the overall reconstruction/realignment of the intersection for the Pennsylvania Department of Transportation Engineering District 6-0. This project is located in Lower Providence Township, Montgomery County (USGS PA Collegeville Quadrangle—Latitude: 40.13376 N, Longitude; 75.42597 W).

E0804221-009. Aqua Pennsylvania, Inc., 139 Center Street, Sayre, PA 18840. Cabinet Works Water Main Replacement in Athens Township, Bradford County, ACOE Baltimore District (Sayre, PA Quadrangle; Latitude: 41° 59` 19"; Longitude: -76° 34` 42").

 To construct approximately 1,200 ft. of 12-inch ductile iron pipe (DIP) water main line that will be supplying residences along King Lane in Athens Township. The proposed main replacement will require the crossing of a wetland along UNT Chemung River (WWF-MF).

 The proposed impact of a PEM wetland is 0.44 acre. The crossing method to be used for the wetland crossing is open-cut trenching. Permanent and temporary impacts for the wetland are proposed as part of this project

 The total disturbed area along the watercourses is 0.05 acre.

 A PNDI search determined no species of concern to be present.

E1904221-001. Borough of Berwick, 1800 N. Market Street, Berwick, PA 18603. Heights Road Crossing in Berwick Borough, Columbia County, ACOE Baltimore District (Berwick, PA Quadrangle; Latitude: 41° 04` 19"; Longitude: -76° 14` 43").

 This project involves the replacement of a culvert crossing that carries Heights Road, a gravel road, over Glen Brook (HQ-CWF) in the Borough of Berwick, Columbia County. Heights Road extends into Briar Creek Township, where it is closed to traffic from the opposite direction. This crossing is therefore the only access to properties across Glen Brook and no other practical alternative is available.

 The total disturbed area along the watercourse is 0.02 acre. Proposed wetland impacts are 0.002 acre.

 A PNDI search determined no species of concern to be present.

E0603220-033. UGI Utilities, 1 UGI Drive, Denver, PA 17517. Chapel Hill Pipeline Exposure in Spring Township, Berks County, ACOE Philadelphia District (Sinking Spring, PA Quadrangle; Latitude: 40° 15` 59"; Longitude: -76° 02` 10").

 To restore protective cover to the exposed portion of an 8" gas line. The pipeline is approximately 4-ft in length and rests on the streambed of UNT Little Muddy Creek (TSF). The placement of repair materials to restore protective cover will raise the streambed approximately 4". To ensure aquatic life passage, the proposed activities incorporate a gradual increase in streambed elevation.

 The total disturbed area along the watercourse is 0.02 acre.

 A PNDI search determined possible bog turtle habitat to be present. An Avoidance Measure is proposed by the applicant. Wetlands have been delineated and marked on site.

 To construct a parking lot involving grading and fill affecting an area measuring approximately 360 feet wide by 150 feet long by approximately 2 feet high and disturbing a total of approximately 1.3 acres within the FEMA 100-year flood plain of the Shenango River at Riverside Park along Alan Avenue north of SR 358. The project has associated NPDES Construction Stormwater PAG-02 and Chapter 105 BWEW GP-04/GP-05 authorizations through the Mercer County Conservation District.

E3206121-001, PADOT, Engineering District 10-0, 2550 Oakland Avenue, Indiana, PA 15701. SR 0954 Section 457 Windows Bridge, in West Mahoning Township, Indiana County, ACOE Pittsburgh District (Plumville, PA Quadrangle N: 40°, 51`, 50.50"; W: 79°, 09`, 45.17").

 To remove the existing structure and to construct and maintain a 45`-10 3/4" long precast concrete box culvert having a span of 14`-0" and a rise of 6`-6" (5`-6" effective with baffles) on SR 0954, Section 457, Segment 0620 over UNT Little Mahoning Creek in West Mahoning Township, Indiana County resulting in 82 linear feet of permanent stream impacts and 74 linear feet of additional temporary stream impacts, and 0.077-ac of permanent wetland impacts and 0.173-ac of temporary wetland impacts to accommodate roadway fill slope adjustments. Permanent wetland impacts will be mitigated by debiting from the existing Indiana County wetland bank owned by the applicant.
