Geisinger Health Plan--Individual, Grange and Association Rate Filing; Rate Filing

   Geisinger Health Plan submitted form and rate filings for the Individual, Grange and Association to make benefit changes. The rates will remain at the current approved level. The following benefit limitations and exclusions will be implemented:

   *  PT/OT/ST--Change 20 visits per year for each therapy to 45 combined dates per year.

   *  Ostomy Supplies--will be covered for Ostomy patients only.

   *  Xolair--will be covered under medical benefit only and not through Rx benefit.

   *  Contraceptives--will not be covered.

   *  Organ Transplant--will cover patients up to $5,000 per transplant for travel/lodging/meals.

   *  Subutex and Suboxone Drugs--will be removed from the Rx formulary and excluded from the Rx rider.

   *  Select ''specialty'' Drugs--a copay of $50 per injection or infusion will be applied up to $1,200 out-of-pocket maximum. A list of the specialty drugs are available with the filing on the website.

   An effective date of April 1, 2008, is requested.

   Unless formal administrative action is taken prior to March 27, 2008, the subject filing may be deemed approved by operation of law.

   A copy of the filing is available on the Insurance Department's (Department) website at www.ins.state. pa.us Under the Quick Links section, click on the link ''Rate Filings Published in the PA Bulletin.''

   Copies of the filing are available for public inspection during normal working hours, by appointment, at the Department's Harrisburg Regional Office.

   Interested parties are invited to submit written comments, suggestions or objections to Rashmi Mathur, Actuary, Bureau of Accident and Health Insurance, Insurance Department, 1311 Strawberry Square, Harrisburg, PA 17120, within 30 days after publication of this notice in the Pennsylvania Bulletin.
