Service of Notice of Motor Carrier Applications

 The following temporary authority and/or permanent authority applications for the right to render service as a common carrier or contract carrier in this Commonwealth have been filed with the Pennsylvania Public Utility Commission. Formal protests and petitions to intervene must be filed in accordance with 52 Pa. Code (relating to public utilities). A protest shall indicate whether it applies to the temporary authority application, the permanent authority application, or both. Filings must be made with the Secretary, Pennsylvania Public Utility Commission, P. O. Box 3265, Harrisburg, PA 17105-3265, with a copy served on the applicant by April 21, 2014. Documents filed in support of the applications are available for inspection and copying at the Office of the Secretary between 8 a.m. and 4:30 p.m., Monday through Friday, and at the business address of the respective applicant.

Applications of the following for approval to begin operating as common carriers for transportation of persons as described under each application.

A-2013-2398919. Cloud 9 Transportation, Inc. (1000 Jefferson Avenue, Scranton, Lackawanna County, PA 18509) for the right to transport, as a common carrier, by motor vehicle, persons in group and party service, in vehicles seating 11 to 15 passengers, including the driver, from points in the Counties of Bradford, Carbon, Columbia, Lackawanna, Lehigh, Luzerne, Monroe, Pike, Schuylkill, Sullivan, Susquehanna, Wayne and Wyoming, to points in Pennsylvania, and return. A-2014-2403919. McCandless-Franklin Park Ambulance Authority (P. O. Box 1, Ingomar, Allegheny County, PA 15127) persons in paratransit service, limited to persons requiring wheelchair or stretcher van service, from points in Allegheny County, to points in Pennsylvania, and return.

A-2014-2406620. Joyce Transportation, LLC (292 Buckhead Lane, Douglassville, Berks County, PA 19518) persons in paratransit service, limited to persons with physical or mental disabilities which prevent them from operating motor vehicles, from points in the Counties of Berks and Montgomery, to points in Pennsylvania, and return.

A-2014-2407540. Sherman Stat Transport, LLC (2201 Highland Road, Hermitage, Mercer County, PA 16148) persons in paratransit service, limited to persons with physical or mental disabilities which prevent them from operating motor vehicles, from points in Mercer County, to points in Pennsylvania, and return.

A-2014-2405375. Hill Investments, LLC (25 Bala Avenue, Bala Cynwyd, Montgomery County, PA 19004) household goods in use, between points in the Counties of Delaware and Montgomery, and the City and County of Philadelphia.

Applications of the following for approval to begin operating as contract carriers for transportation of persons as described under each application.

A-2014-2402998. EZ Ride, LLC (P. O. Box 8083, Gaithersburg, MD 20898) for the right to begin to transport, as a contract carrier for medicaid in this Commonwealth, by motor vehicle, persons in paratransit service, between points in the City and County Philadelphia, to points in Pennsylvania, and return.

A-2014-2408145. Corporate Enterprise Solutions, LLC (412 North Douglas Street, Philadelphia, PA 19132) for the right to begin to transport, as a contract carrier, by motor vehicle, persons for Broad Wellness Chiropractic Center between points in the County of Philadelphia.

Applications of the following for the approval of the right and privilege to discontinue/abandon operating as common carriers by motor vehicle and for cancellation of the certificate of public convenience as described under each application.

A-2014-2405396. Hanna Transfer Company—North (1300 West First Street, Oil City, Venango County, PA 16301) for the discontinuance of service and cancellation of their certificate as a household goods carrier, to transport, by motor vehicle, household goods in use, between points in the City of Oil City, Venango County and within 25 miles by the usually traveled highways of the limits of said city and from points in the said city and territory, to other points in Pennsylvania, and vice versa, and between points in the County of Warren, and from points in the said county, to points in Pennsylvania, and vice versa.

A-2014-2407231. Betty M. Hamilton (3380 Gap Road, Allenwood, Lycoming County, PA 17810) discontinuance of service and cancellation of her certificate, as a common carrier, persons in paratransit service, limited to persons whose personal convictions prevent them from owning or operating motor vehicles, from points in the Counties of Clinton and Union to points in Pennsylvania, and return.
