Service of Notice of Motor Carrier Applications

 The following temporary authority and/or permanent authority applications for the right to render service as a common carrier or contract carrier in this Commonwealth have been filed with the Pennsylvania Public Utility Commission. Formal protests and petitions to intervene must be filed in accordance with 52 Pa. Code (relating to public utilities). A protest shall indicate whether it applies to the temporary authority application, the permanent authority application, or both. Protests may only be filed in the event that there is evidence that the applicant lacks fitness. Protests based on endangering or impairing operations of an existing carrier will not be honored. Filings must be made with the Secretary, Pennsylvania Public Utility Commission, P.O. Box 3265, Harrisburg, PA 17105-3265, with a copy served on the applicant by July 30, 2018. Documents filed in support of the applications are available for inspection and copying at the Office of the Secretary between 8 a.m. and 4:30 p.m., Monday through Friday, and at the business address of the respective applicant.

Applications of the following for approval to begin operating as common carriers for transportation of persons as described under each application.

A-2018-3002441. Guardian Transport Services, LLC (8796 Route 219, Brockway, Jefferson County, PA 15824) in paratransit service, from points in the Counties of Erie, Clarion, Jefferson, Clearfield, Indiana, Cambria and Blair, to points in Pennsylvania, and return.

A-2018-3002616. Fenrick Enterprises, LLC, t/a Tina's Taxi And Delivery (1739-A Butler Pike, Mercer, Mercer County, PA 16137) in paratransit service, limited to persons whose personal convictions prevent them from owning or operating motor vehicles, from points in the Counties of Mercer, Butler, Lawrence, Venango and Crawford to points in Pennsylvania, and return.

A-2018-3002810. TM Transportation, LLC (701 Disston Street, Apartment 3, Philadelphia, PA 19111) in paratransit service, from points in the City and County of Philadelphia, to points in Pennsylvania, and return.

A-2018-3002944. Delmar S. Woodring, t/a SV trnsprtr (3850 Kettle Road, Altoona, Blair County, PA 16001) in paratransit service, limited to persons whose personal convictions prevent them from owning or operating motor vehicles, from points in Blair County, to points in Pennsylvania, and return.

Application of the following for amendment to the certificate of public convenience approving the operation of motor vehicles as common carriers for transportation of persons as described under the application.

A-2018-3002771. Pottstown Memorial Ambulance Company, LLC, t/a Pottstown Medical Transport (1600 East High Street, Pottstown, Montgomery County, PA 19464) in paratransit service, between Brandywine Hospital, Jennersville Hospital, Phoenixville Hospital, Reading Hospital and Pottstown Hospital, nursing facilities, hospitals, surgical, diagnostic or similar clinics, dialysis centers, physician offices or patient's residences, or both, within the Counties of Berks Chester and Montgomery. Applications of the following for the approval of the right and privilege to discontinue/abandon operating as common carriers by motor vehicle and for cancellation of the certificate of public convenience as described under each application.

A-2018-3002707. James Carl Cisney (7 Lepore Drive, Lancaster, Lancaster County, PA 17602) for the discontinuance of service and cancellation of its certificate, as a common carrier, by motor vehicle, at A-00099130, F.3, authorizing the transportation of persons, in paratransit service, from points in the Borough of Quarryville, and within an airline distance of 6 statute miles of the limits of said borough, to points in Pennsylvania, and vice versa.

A-2018-3002730. O'Malley Moving & Hauling, Inc. (3 Raymond Drive, Suite 101, Havertown, Delaware County, PA 19083) discontinuance of service and cancellation of its certificate, to transport, as a common carrier, by motor vehicle, household goods in use, between points in the Counties of Philadelphia, Delaware, Chester, Montgomery and Bucks, as more thoroughly described in the original ordering paragraphs at A-00099724.
