AIR QUALITY
PLAN APPROVAL AND OPERATING PERMIT APPLICATIONS
NEW SOURCES AND MODIFICATIONS The Department of Environmental Protection (Department) has developed an ''integrated'' plan approval, State operating permit and Title V operating permit program. This integrated approach is designed to make the permitting process more efficient for the Department, the regulated community and the public. This approach allows the owner or operator of a facility to complete and submit all the permitting documents relevant to its application one time, affords an opportunity for public input and provides for sequential issuance of the necessary permits.
The Department has received applications for plan approvals and/or operating permits from the following facilities.
Copies of the applications, subsequently prepared draft permits, review summaries and other support materials are available for review in the regional office identified in this notice. Persons interested in reviewing the application files should contact the appropriate regional office to schedule an appointment.
Persons wishing to receive a copy of a proposed plan approval or operating permit must indicate their interest to the Department regional office within 30 days of the date of this notice and must file protests or comments on a proposed plan approval or operating permit within 30 days of the Department providing a copy of the proposed document to that person or within 30 days of its publication in the Pennsylvania Bulletin, whichever comes first. Interested persons may also request that a hearing be held concerning the proposed plan approval and operating permit. Comments or protests filed with the Department regional offices must include a concise statement of the objections to the issuance of the Plan approval or operating permit and relevant facts which serve as the basis for the objections. If the Department schedules a hearing, a notice will be published in the Pennsylvania Bulletin at least 30 days prior the date of the hearing.
Persons with a disability who wish to comment and require an auxiliary aid, service or other accommodation to participate should contact the regional office identified before the application. TDD users should contact the Department through the Pennsylvania AT&T Relay Service at (800) 654-5984.
Final plan approvals and operating permits will contain terms and conditions to ensure that the source is constructed and operating in compliance with applicable requirements in 25 Pa. Code Chapters 121--143, the Federal Clean Air Act (act) and regulations adopted under the act.

62-162B: Elkhorn Field Services (Zimmerman Hill Road, Warren, PA 16365) for construction of a new refrigeration compressor and a larger glycol dehydration system at the Keystone Gas Processing Plant in Pleasant Township, Warren County. The facility currently has a State-only Operating Permit.

18-00004A: Armstrong World Industries, Inc. (325 Eagle Valley Road, Beech Creek, PA 16822) for printing of nonflooring materials in the existing 9-station rotogravure printing press in Beech Creek Township, Clinton County. The air contaminant emissions from the printing press will be controlled by an existing regenerative thermal oxidizer.
The proposed modification will utilize inks and cleaning solvents for the printing of nonflooring materials that contain VOCs and HAPs. The proposed modification will also limit the facility as a whole to emit less than 7 tons of individual HAP and 10 tons of combined HAPs in any 12-consecutive month period. The VOC emissions from the proposed modification will be included within the facility's existing emission limitation of 250 tons of VOC in any 12-consecutive month period. The facility is also required to keep records on a monthly basis and submit semi-annual reports to verify compliance with the facility-wide VOC and HAPs emission limitations.
The Department of Environmental Protection's (Department) review of the information provided by Armstrong World Industries, Inc. indicates that the proposed modification will comply with all air quality requirements, including the best available technology requirement of 25 Pa. Code §§ 127.1 and 127.12 pertaining to air contamination sources and the emission of air contaminants. The proposed modification will also comply with the applicable requirements of 40 CFR Part 60, Subpart FFF (Standards of Performance for Flexible Vinyl and Urethane Coating and Printing), 60.580--60.585. Based on these findings, the Department intends to approve the application and issue plan approval for the proposed modification, including printing of non-flooring materials in the existing 9-station rotogravure printing press. Additionally, if the Department determines that the facility is operating in compliance with the plan approval conditions, the conditions established in the plan approval will be incorporated into the State-only operating permit No. 18-00004 by means of an administrative amendment under 25 Pa. Code § 127.450.
The following is a summary of the conditions that the Department proposes to place in the plan approval to ensure compliance with all applicable regulatory requirements.
1. Under the Best Available Technology (BAT) provisions in 25 Pa. Code §§ 127.1 and 127.12, the emission of any individual HAP from all sources at the facility shall not exceed 7.0 tons in any 12-consecutive month period. The allowable individual HAPs are toluene and other alkyl aromatics (family), methylene diphenyl diisocyanate and other diisocyanates (family), methyl isobutyl ketone, xylenes, hexane, alcohols (family) and glycol ethers. Additionally, all sources at the facility shall emit no greater than 0.5 ton of any individual HAP that is not listed previously as an allowable individual HAP. The emission of total combined HAPs from all sources at the facility shall not exceed 10.0 tons in any 12-consecutive month period. The permittee shall not emit polychlorobiphenols, mercury, lead, polycyclic organic matter, dioxins or furans from any source at the facility.
2. Under the BAT provisions in 25 Pa. Code §§ 127.1 and 127.12, the permittee shall not process more than 70 million square feet of nonflooring substrate through Source ID P101 in any 12-consecutive month period.
3. Under the BAT provisions in 25 Pa. Code §§ 127.1 and 127.12, the emission of VOCs from all sources at the facility shall not equal or exceed 250 tons in any 12-consecutive month period.
4. The permittee shall keep up-to-date records of Certified Product Data Sheets (CPDSs) that identify the VOC content and HAP content of each VOC and/or HAP containing material used at the facility.
5. The permittee shall keep records of the amount of nonflooring substrate processed through Source ID P101 each month.
6. The permittee shall keep records of the identity, type, and amount of each VOC and/or HAP containing material used each month in association with the printing of nonflooring materials in Source ID P101.
7. Under the BAT provisions in 25 Pa. Code Sections 127.1 and 127.12, Source ID P101 shall not be operated without the simultaneous operation of regenerative thermal oxidizer (ID C01). The emission of VOCs and HAPs from Source ID P101 shall be controlled by ID C01.
8. Under the BAT provisions in 25 Pa. Code §§ 127.1 and 127.12, the VOC and HAP emissions from the printing of nonflooring materials in Source ID P101 shall be captured with an efficiency of 100% and exhausted to ID C01. Compliance with the requirement specified in this streamlined permit condition assures compliance with the provisions in 25 Pa. Code § 129.67 and 40 CFR 60.582(a)(2).
9. Under the BAT provisions in 25 Pa. Code §§ 127.1 and 127.12, ID C01 shall destroy the VOC and HAP emissions ducted to it from all sources at the facility associated with the production area (solvent process area) at a destruction efficiency of no less than 95% (by weight). Compliance with the requirement specified in this streamlined permit condition assures compliance with the provisions in 25 Pa. Code § 129.67 and 40 CFR Part 60.582(a)(2).
10. Under the BAT provisions in 25 Pa. Code §§ 127.1 and 127.12, within 120 days of the issuance of this plan approval, the permittee shall perform stack testing upon the inlet and outlet of ID C01 while ID C01 is being used to control emissions from the printing of nonflooring substrates in Source ID P101 to determine the VOC/HAP destruction efficiency of ID C01 to verify compliance with the destruction efficiency limitation. The testing shall be performed while Source ID P101 is operating at a representative rate of production, using test methods and procedures approved by the Department. During the stack testing, the temperature set-point of the combustion chamber of ID C01 shall be 1,450°F and the actual instantaneous temperature of the combustion chamber of ID C01 shall not go below 1,400°F. Compliance with the requirement for the timing of the source testing specified in this streamlined permit condition assures compliance with the provisions in 40 CFR 60.8(a) and 583(d).
11. Under the BAT provisions in 25 Pa. Code §§ 127.1 and 127.12, the permittee shall operate ID C01 at a temperature of at least 1,400°F.
12. Under the BAT provisions in 25 Pa. Code §§ 127.1 and 127.12, ID C01 shall be equipped with two thermocouples to monitor and record the temperature of the combustion chamber of ID C01 on a continuous basis. Compliance with the temperature monitoring and recording requirement specified in this streamlined permit condition assures compliance with the provision in 40 CFR 60.584(b).
13. Under the BAT provisions in 25 Pa. Code §§ 127.1 and 127.12, the permittee shall determine the VOC destruction efficiency of ID C01 on a quarterly basis using a total hydrocarbon analyzer. The VOC destruction efficiency shall be determined by measuring the VOC concentration at the inlet and outlet of the incinerator. A zero and span calibration shall be performed on the total hydrocarbon analyzer immediately prior to the testing of the inlet of ID C01, immediately prior to the testing of the outlet of ID C01, and immediately after the testing of the outlet of ID C01.
14. The permittee shall keep records of the supporting calculations for VOC and HAP emissions from all sources at the facility on a monthly basis and shall verify compliance with the VOC and HAP emission limitations in any 12-consecutive month period.
15. The permittee shall keep records of the dates and results of the VOC concentration stack tests, including total hydrocarbon analyzer tests with calibration data, and shall verify compliance with the VOC destruction efficiency requirement specified for ID C01.
16. The permittee shall keep records of the temperature of ID C01 on a monthly basis and shall verify compliance with the operating temperature requirement of 1,400°F specified for ID C01.
17. The permittee shall keep records of the dates and results of the temperature device accuracy tests on a monthly basis and shall verify compliance with the temperature device accuracy requirement specified for ID C01.
18. The permittee shall submit reports to the Department on a semi-annual basis that include the supporting calculations used to verify compliance with the VOC destruction efficiency requirement specified for ID C01.
19. The permittee shall submit reports to the Department on a semi-annual basis that include the supporting calculations used to verify compliance with the VOC and HAP emission limitations for all sources at the facility in any 12-consecutive month period.
20. The permittee shall test the thermocouples associated with ID C01 on a monthly basis and maintain an accuracy of plus or minus 2°F. The permittee shall utilize approved QA/QC practices that are adequate to ensure continuing validity of data and proper performance of the thermocouples. Compliance with the requirement specified in this streamlined permit condition assures compliance with the provision in 40 CFR 60.584(b)(1).
21. Under the BAT provisions in 25 Pa. Code §§ 127.1 and 127.12, the permittee shall ensure that ID C01 has reached its minimum operating temperature before placing any affected source on line.
22. Under the BAT provisions in 25 Pa. Code §§ 127.1 and 127.12, the permittee shall have an alarm system that alerts when the temperature of ID C01 falls below 1,423°F. This alarm system shall automatically shutdown all processes exhausted to ID C01 after no more than a 10-minute delay.
23. The permittee shall record all instances that the temperature of ID C01 falls below 1,423°F while the emissions from any source at the facility are being controlled by ID C01. Compliance with the requirement specified in this streamlined permit condition assures compliance with the provision in 40 CFR 60.584(b)(2).
24. The permittee shall submit reports to the Department on a semi-annual basis that include the records of all instances that the temperature of ID C01 falls below 1,423°F while the emissions from any source at the facility are being controlled by ID C01. Compliance with the requirement specified in this streamlined permit condition assures compliance with the provision in 40 CFR 60.585(b)(3).
25. Source ID P101 is subject to 25 Pa. Code § 129.67. The permittee shall comply with all the applicable requirements specified in 25 Pa. Code § 129.67.
26. Nonflooring substrates printed from Source ID P101 are subject to 40 CFR Part 60, Subpart FFF. The permittee shall comply with all the applicable requirements specified in 40 CFR 60.580--60.585.
27. The Request for Determination of Requirement of Plan Approval/Operating Permit (RFD) forms approved for printing on polyvinyl alcohol (PVOH) film on March 4, 2005, and for printing on polyester (''Mylar'') film on April 5, 2005, including all conditions of approval, are superceded by this plan approval.
28. All conditions contained in TVOP 18-00004 remain in effect unless superseded or amended by conditions contained in this plan approval. If there is a conflict between a condition contained in this plan approval and a condition contained in TVOP 18-00004, the permittee shall comply with the condition contained in this plan approval.

The operating permit will contain appropriate conditions designed to keep the facility operating below Title V thresholds for criteria pollutants and within all other applicable air quality requirements.
07-05034: Department of Military and Veterans Affairs--Hollidaysburg Veterans Home (P. O. Box 319, Hollidaysburg, PA 16648-0319) for operation of the long-term nursing care facility in Allegheny Township, Blair County. The facility is a major source that primarily emits nitrogen oxides (NOx) through the operation off one natural gas and two natural gas/No. 2 oil fired boilers along with five No. 2 fuel oil operated electric generator sets. This State-only (Synthetic Minor) Operating Permit will limit actual emissions for criteria pollutants below Title V thresholds by limiting the annual operating time of the emergency generators. The operating permit will contain appropriate conditions designed to keep the facility operating within all other applicable air quality requirements. This is a renewal of the State-only (Synthetic Minor) Operating Permit issued in 2001.

57-00002: Haines and Kibblehouse, Inc.--Dushore Construction Materials Division (P. O. Box 196, Skippack, PA 19474) for operation of a stone crushing facility on Route 220 in Cherry Township, Sullivan County.
The respective plant incorporates various pieces of stone crushing, screening, conveying, and the like equipment as well as an 890 horsepower diesel engine, a 270 horsepower diesel engine and several minor air contamination sources such as a diesel fuel storage tank. The fugitive PM emissions, including PM10 from the stone crushing, screening, conveying, and the like equipment are controlled by a water spray dust suppression system. The fugitive PM and PM10 emissions resulting from the use of site roadways are controlled by a water truck. The facility is not expected to emit more than 14.02 tons of PM, 6.56 tons of PM10, 10.46 tons of NOx, 2.8 tons of CO, 2.76 tons of SOx and 1.31 tons of total hydrocarbons per year.
The facility is not a major (Title V) facility for any air contaminant.
The Department of Environmental Protection (Department) proposes to incorporate into the operating permit to be issued conditions requiring compliance with all applicable regulatory requirements pertaining to air contamination sources and the emission of air contaminants as well as conditions previously established in Plan Approval 57-310-001, issued on August 29, 2005.
These previously-established conditions include:
1. Conditions limiting the operation of the stone crushing, screening, conveying, and the like equipment and the two associated diesel engines to no more than 2,000 hours in any 12-consecutive month period.
2. A condition requiring the stone crushing, screening, conveying, and the like equipment to be equipped with a water spray dust suppression system which shall incorporate at least six spray bars with at least three spray nozzles each.
3. A condition requiring the water spray dust suppression system to incorporate strainers to prevent nozzle pluggage.
4. A condition requiring a water truck equipped with a spray bar to be kept on site at all times and used, as needed, for the control of fugitive particulate matter from plant roadways and stockpile areas.
5. Conditions prohibiting the operation of the stone crushing, screening, conveying, etc. equipment if either the water spray dust suppression system or the water truck is incapable of operation.
6. A condition requiring the plant to incorporate the hydrants or hose connections as are needed to permit the use of hoses to control fugitive particulate matter from material stockpiles.
7. Conditions limiting the sulfur content of the diesel fuel used in the diesel engines to .3% by weight.
8. A condition limiting the NOx, CO and total hydrocarbon emissions from the 890 horsepower diesel engine to 2.77, .23 and .32 grams per horsepower-hour, respectively.
9. A condition limiting the NOx, CO and total hydrocarbon emissions from the 270 horsepower diesel engine to 8.44, 3.03 and 1.14 grams per horsepower-hour, respectively.
10. A condition requiring the injection timing of the 270 horsepower diesel engine to be retarded at least 4° relative to standard timing.
11. A condition requiring the maintenance of records of the number of hours the stone crushing plant operates each month as well as the number of hours each of the two diesel engines operates each month.
The Department additionally proposes to incorporate into the operating permit to be issued a new condition requiring the maintenance of records of the identity and amount of aerosol degreasers used at the facility each month.

37-00300: North American Forgemasters Co. (710 Moravia Street, New Castle PA 16101-3948) a Natural Minor operating permit for manufacture of hot-formed forged products in city of New Castle, Lawrence County.
25-00931: Deptment of Corrections--Albion SCI (10745 Route 18, Albion, PA 16475) for the reissuance of State-only Operating Permit No. 25-00931 issued to the Albion State Correctional Institute located in Albion Borough, Erie County. The air contamination sources located at the facility include various boilers, electric generators and painting booths.
COAL AND NONCOAL MINING ACTIVITY APPLICATIONS Applications under the Surface Mining Conservation and Reclamation Act (52 P. S. §§ 1396.1--1396.19a); the Noncoal Surface Mining Conservation and Reclamation Act (52 P. S. §§ 3301--3326); The Clean Streams Law (35 P. S. §§ 691.1--691.1001); the Coal Refuse Disposal Control Act (52 P. S. §§ 30.51--30.66); and The Bituminous Mine Subsidence and Land Conservation Act (52 P. S. §§ 1406.1--1406.21). Mining activity permits issued in response to applications will also address the applicable permitting requirements of the following statutes: the Air Pollution Control Act (35 P. S. §§ 4001--4015); the Dam Safety and Encroachments Act (32 P. S. §§ 693.1--693.27); and the Solid Waste Management Act (35 P. S. §§ 6018.101--6018.1003).
The following permit applications to conduct mining activities have been received by the Department of Environmental Protection (Department). A copy of an application is available for inspection at the district mining office indicated before an application. Where a 401 Water Quality Certification is needed for any aspect of a particular proposed mining activity, the submittal of the permit application will serve as the request for certification.
Written comments, objections or requests for informal conferences on applications may be submitted by any person or any officer or head of any Federal, State or local government agency or authority to the Department at the district mining office indicated before an application within 30 days of this publication, or within 30 days after the last publication of the applicant's newspaper advertisement, as provided by 25 Pa. Code §§ 77.121--77.123 and 86.31--86.34.
Where any of the mining activities listed will have discharges of wastewater to streams, the Department will incorporate NPDES permits into the mining activity permits issued in response to these applications. NPDES permits will contain, at a minimum, technology-based effluent limitations as identified in this notice for the respective coal and noncoal applications. In addition, more restrictive effluent limitations, restrictions on discharge volume or restrictions on the extent of mining which may occur will be incorporated into a mining activity permit, when necessary, for compliance with water quality standards (in accordance with 25 Pa. Code Chapters 93 and 95). Persons or agencies who have requested review of NPDES permit requirements for a particular mining activity within the previously mentioned public comment period will be provided with a 30-day period to review and submit comments on the requirements.
Written comments or objections should contain the name, address and telephone number of the person submitting comments or objections; the application number; and a statement of sufficient detail to inform the Department on the basis of comment or objection and relevant facts upon which it is based. Requests for an informal conference must contain the name, address and telephone number of requestor; the application number; a brief summary of the issues to be raised by the requestor at the conference; and a statement whether the requestor wishes to have the conference conducted in the locality of the proposed mining activities.

FEDERAL WATER POLLUTION CONTROL ACT, SECTION 401 The following permit applications, requests for Environmental Assessment approval and requests for 401 Water Quality Certification have been received by the Department of Environmental Protection (Department). Section 401 of the Federal Water Pollution Control Act (FWPCA) (33 U.S.C.A. § 1341) requires the State to certify that the involved projects will not violate the applicable provisions of sections 301--303, 306 and 307 of the FWPCA (33 U.S.C.A. §§ 1311--1313, 1316 and 1317) as well as relevant State requirements.
