STATE SYSTEM OF HIGHER EDUCATION

Request for Proposals

   Indiana University of Pennsylvania of the State System of Higher Education invites interested professionals to submit proposals for an Open-ended Professional Design Services Contract to be used on various projects at the university. The university is seeking two predominantly engineering teams and one architectural/engineering team to provide open-ended services. The firms and their respective consultants shall be capable of providing, at a minimum, architectural, structural, HVAC, plumbing, electrical, landscape architectural and survey services on an individual project basis. The university desires that at least one of the teams have multi-disciplinary capabilities including environmental services such as testing, field survey and supervision of remediation activities.

   Interested professionals are to submit the information required under the following section entitled ''Instructions for Filing Application.''

   Proposals must be received on or before the close of business August 19, 1997, addressed to Slating Committee, Engineering and Construction Group Office, Indiana University of Pennsylvania, Robert Shaw Building, 650 S. 13th Street, Indiana, PA 15705-1087.

Instructions for Filing Application

   Professionals will not be considered by the System Slating Committee until all of the following requirements are met.

   (a)  A Request for Proposal 01-97 has been requested in writing or FAX (412) 357-6480 from the Engineering and Construction Group Office outlining the requirements. The professional is to respond to the RFP as well as submit the following:

   (b)  Form 150 (Rev 4/94), or Standard Form 254 not more than 1 year old as of the deadline stated above, must be filed with the University for the requesting professional firm and the designated key consultants listed on the requesting professional firm's application (Form 150-S, or Standard Form 255). Forms may be obtained upon request to the University.

   (c)  The Professional must submit six copies of either form (Form 150 and 150-S or SF 254 and 255). When appropriate, the Professional shall supply photographs showing a maximum of two different views of each of the three projects described in Question 13, Page 4 of the application (Form 150-S or Question 10 of SF-255). The identifications and appropriate supportive information concerning each photograph shall include the name, project title, location and the name of the Professional of Record.

   Renderings and brochures will not be accepted in lieu of photographs. The photographs shall be attached to each copy of the application (Form 150-S). The pages of each copy of the Form 150-S (REV. 4/94) must be stapled with photographs included and each copy must remain free from the other and not bound in any way to any other documentation or bound in a binder of any type. Subsequent to the public meeting of the Slating Committee, the university will return photographs submitted with applications to those firms that request it, and provide a self-addressed stamped envelope in which to return the photographs. The most qualified firms will be slated for consideration by the University Selections Board.

   (d)  The University Selections Board, may at its discretion, establish interviews with any or all of the professionals who have requested consideration for appointment. If an interview is required, the professional will be notified by the Board as to the date, time and location.

   (e)  Additional information, in writing, may be requested by the Slating Committee as required. Professional agrees that they shall comply with the terms of the Agreement and specifically as it relates to the Professional Liability Insurance and the General Liability Insurance Requirements.

   The Slating Committee encourages responses from small firms, minority firms, women-owned firms and firms who have not previously performed System work, and will consider Joint Ventures, which will enable them to participate in this program.

   Applications submitted are subject to review by the Slating Committee. The Slating Committee and Selections Board disclaim any liability whatsoever as to their review of the applications submitted and in formulating their recommendations for selection. Recommendations for selection made by the Committee or Board shall be final under the act of November, 1982 (P. L. 660, No. 188 as amended).
