FEDERAL WATER POLLUTION CONTROL ACT, SECTION 401

   The following permit applications and requests for Environmental Assessment approval and requests for Water Quality Certification have been received by the Department of Environmental Protection. Section 401 of the Federal Water Pollution Control Act (FWPCA) (33 U.S.C.A. § 1341(a)), requires the State to certify that the involved projects will not violate the applicable provisions of sections 301--303, 306 and 307 of the FWPCA (33 U.S.C.A. §§ 1311--1313, 1316 and 1317) as well as relevant State requirements. Initial requests for 401 Water Quality Certification will be published concurrently with the permit application. Persons objecting to approval of a request for certification under section 401 or to the issuance of a Dam Permit or Water Obstruction and Encroachment Permit or the approval of an Environmental Assessment must submit any comments, suggestions or objections within 30 days of the date of this notice as well as any questions to the office noted before the application. Comments should contain the name, address and telephone number of the person commenting, identification of the certification request to which the comments or objections are addressed and a concise statement of comments, objections or suggestions including the relevant facts upon which they are based.

   The Department may conduct a fact-finding hearing or an informal conference in response to comments if deemed necessary. Each individual will be notified, in writing, of the time and place of a scheduled hearing or conference concerning the certification request to which the comment, objection or suggestion relates. Maps, drawings and other data pertinent to the certification request are available for inspection between the hours of 8 a.m. and 4 p.m. on each working day at the office noted before the application.

   Persons with a disability who wish to attend the hearing and require an auxiliary aid, service or other accommodation to participate in the proceedings should contact the specified program. TDD users may contact the Department through the Pennsylvania AT&T Relay Service at (800) 654-5984.

   E64-222. Waymart Area Authority, P. O. Box 3300, Waymart, PA 18472-0224, in Waymart Borough and Canaan Township, Wayne County, U. S. Army Corps of Engineers, Philadelphia District.

   To construct and maintain two utility line crossings of wetlands, in the drainage basin of Van Auken Creek (HQ-CWF), for the purpose of constructing Phase II of a wastewater sewerage facilities upgrade project. Approximately 325 L.F. of a 15-inch interceptor gravity sewer will cross ''Wetland A'' and approximately 730 L.F. of an 8-inch effluent force main will cross ''Wetland B''. Other utility line crossings associated with the overall project are being authorized under the Department's General Permit BDWM-GP-5. The project is located to the east of S.R. 0296 and to the north of Lake Ladore. (Waymart, PA Quadrangle N:  14.1 inches; W:  4.1 inches).

   E45-315. Palmer Township, P. O. Box 3039, Palmer, PA 18043-3039, in Palmer Township, Northampton County, U. S. Army Corps of Engineers, Philadelphia District.

   To construct and maintain a pedestrian bridge having a single span of 40 feet and an underclearance of approximately 6 feet across Shoeneck Creek (wWF). The project is located in Wolf's Run Subdivision between Camelot Drive and Melissa Drive. (Nazareth, PA Quadrangle N:  16.5 inches; W:  1.3 inches).

   E08-384. Pennsylvania Department of Transportation, District 3-0, 715 Jordan Avenue, Montoursville, PA 17754-0218, Bridge Replacement in Towanda Township, Bradford County, ACOE Baltimore District (Monroeton, PA Quadrangle N:  21.00 inches; W:  8.13 inches).

   To remove the existing through-truss one lane bridge which has a clear span of 181 feet and an out-to-out width of 18 feet and to construct and maintain a twin span composite prestressed I-beam bridge with a total span of 249 feet, a normal span of 120 feet, an underclearance of 21.4 feet on a skew of 75° over Towanda Creek on SR 2024, Section 0001 and to construct a temporary stream crossing approximately 300 feet northwest of the intersection of SR 2024 with Tip Top Road. The project will not impact wetlands while impacting approximately 200 feet of waterway. Towanda Creek is a warm water fisheries stream.

   E18-328. Richard J. Noll, 6965 Nittany Valley Drive, Mill Hall, PA 17751. Private Driveway Culvert, in Beech Creek Township, Clinton County, ACOE Baltimore District (Beech Creek, PA Quadrangle N:  19.8 inches; W:  8.8 inches).

   To maintain an I-beam timber deck bridge with reinforced concrete abutments and wingwalls with a maximum waterway depth of 6.8 feet and hydraulic opening of 121 square feet over Masden Run off Masden Hollow Road approximately 2.3 miles from the intersection of Masden Hollow Road with SR 0150 in Beech Creek Township, Clinton County. The project will not impact wetlands while impacting approximately 20 feet of waterway. Masden Run is a cold water fisheries stream.

   E19-217. First Church of Christ, 102 Derrs Road, Bloomsburg, PA 17815. Wetland Excavation and Sewer Line Stream Crossing, in Montour Township, Columbia County, ACOE Baltimore District (Catawissa, PA Quadrangle N:  18.7 inches; W:  16.2 inches).

   To construct and maintain a 25 foot long sewer line crossing of Montour Run, to fill approximately 0.28 acre of existing palustrine emergent wetlands and to construct and maintain a replacement wetland approximately 15 feet in width and spanning the entire southern boundary of the parcel. The project is located on the south side of Montour Run approximately 1,300 feet south of the Valley School in Montour Township, Columbia County. The project will impact approximately 0.28 acre of wetlands while impacting 25 feet of stream. Montour Run is a cold-water fisheries stream.

   E02-1025 A1. City of Pittsburgh, Department of Engineering and Construction, Room 301 City County Building, 414 Grant Street, Pittsburgh, PA 15219. City of Pittsburgh, Allegheny County, ACOE Pittsburgh District.

   To reissue and amend permit E02-1025 which authorized the removal of the existing structure and to construct and maintain a bridge having a span of 35.0 feet and underclearance of 9.87 feet across Saw Mill Run (WWF) located on Edgebrook Avenue near its intersection with Saw Mill Run Boulevard. This amendment will authorize the construction and maintenance of a single span bridge having a span of 33.75 feet and underclearance of 9.8 feet across Saw Mill Run (Pittsburgh West, PA Quadrangle N:  4.9 inches; W:  0.2 inch).

   E04-278. Big Beaver Falls Area School District, Administrative Offices, 820 16th Street, Beaver Falls, PA 15010. Beaver Falls, Beaver County, ACOE Pittsburgh District.

   To place and maintain fill within the left bank floodway of Walnut Bottom Run (WWF) for the purpose of constructing a new baseball and football field and to provide additional parking. The project is located at the existing Beaver Falls Elementary School on the northwest side of the intersection of 8th Avenue and Fifteenth Street (Beaver Falls, PA Quadrangle N:  1.6 inches; W:  10.0 inches).

   E26-290. NWL Company, 1001 LaFayette Drive, Farmington, PA 15437. Wharton Township, Fayette County, ACOE Pittsburgh District.

   To remove the existing culvert to construct and maintain a single 48-inch RCP culvert in an unnamed tributary to Deadman Run (HQ-CWF) and to place and maintain fill in 0.51 acre of wetlands for the purpose of developing an outdoor store. The proposed work includes modifying a nonjurisdictional dam across an unnamed tributary to Deadman Run (HQ-CWF) and a total watercourse impact of 684 feet. The project is located along US Route 40 at its intersection with Smith Schoolhouse Road (Fort Necessity, PA Quadrangle N:  8.3 inches; W:  5.5 inches).

   E63-519. Pennsylvania Department of Transportation, Engineering District 12-0, P. O. Box 459, Uniontown, PA 15401. Morris Township, Washington County, ACOE Pittsburgh District.

   To rehabilitate and maintain the existing S.R. 0018 bridge consisting of inserting a 7.0 foot diameter culvert inside the existing 9.5 foot diameter culvert in an unnamed tributary to Short Creek (TSF). The bridge is located on S.R. 0018, Segment 0030, Offset 0253 (Prosperity, PA Quadrangle N:  1.5 inches; W:  12.6 inches).

   E63-523. Arden Landfill, Inc., Box BC, Arden Station Road, Washington, PA 15301. Chartiers Township, Washington County, ACOE Pittsburgh District.

   To place and maintain fill in 0.4 acre of PF/PEM wetlands associated with an unnamed tributary to Georges Run (WWF) for the purpose of expanding the existing Arden Landfill. The project is located on the west side of Arden Road, approximately 4,500 feet northwest from the intersection of Arden Road and Wolfdale Road (Washington West, PA Quadrangle N:  16.0 inches; W:  3.0 inches).

   E65-791. Eastgate Dodge, Route 30 East, Georges Station Road, Greensburg, PA 15601. Hempfield Township, Westmoreland County, ACOE Pittsburgh District.

   To extend and maintain an existing 72-inch reinforced concrete pipe stream enclosure having a length of 1,000 feet with 390 feet of a 60 inch corrugated metal pipe upstream in an unnamed tributary to Slate Creek (WWF) for the purpose of creating usable surface for the car dealership. The project is located along the east side of S.R. 1053, approximately 500 feet north of its intersection with S.R. 0030 (Latrobe, PA Quadrangle N:  9.7 inches; W:  16.18 inches).

   E10-344, Landmark Properties, Inc., 4848 Route 8, Allison Park, PA 15101. Emeryville Drive Lots 1 and 2 Wetland Fill in Cranberry Township, Butler County, ACOE Pittsburgh District (Mars, PA Quadrangle N:  9.3 inches; W:  14.1 inches).

   To fill 0.13 acre of wetland on Emeryville Drive Lots 1 and 2 for construction of a 5 story office building and associated parking area along Emeryville Drive approximately 0.25 mile east of Margarite Drive. Project includes contribution to the Pennsylvania Wetland Replacement Fund for replacement of impacted wetland.

   The Department of Environmental Protection (Department) has taken the following actions on previously received permit applications and requests for plan approval. The actions are listed in two categories. Section I lists all municipal and industrial permits and Section II lists oil and gas related permits.

   Persons aggrieved by this action may appeal, under section 4 of the Environmental Hearing Board Act (35 P. S. § 7514) and 2 Pa.C.S. §§ 501--508 and 701--704 (relating to the Administrative Agency Law), to the Environmental Hearing Board, Second Floor, Rachel Carson State Office Building, P. O.    NPDES Permit No. PA-0060054, Sewage, Pocono Associates, L.P., 42 Woodland Road, Mount Pocono, PA 18344-9703. This proposed facility is located in Paradise Township, Monroe County.

   Description of Proposed Action/Activity:  Renewal of NPDES Permit to discharge treated sewage to Forest Hills Run.

   NPDES Permit No. PA-0027235, Sewage, Easton Area Joint Sewer Authority, 50-A South Delaware Drive, Easton, PA 18042. This proposed facility is located in City of Easton, Northampton County.

   Description of Proposed Action/Activity:  Renewal of NPDES Permit to discharge treated sewage into the Delaware River.

   Southwest Region:  Water Management Program Manager, 400 Waterfront Drive, Pittsburgh, PA 15222-4745.

   NPDES Permit No. PA0038181. Sewage. Hempfield Township Municipal Authority, R. D. 6, Box 501, Municipal Building, Greensburg, PA 15601 is authorized to discharge from a facility located at New Stanton Water Pollution Control Plant, Hempfield Township, Westmoreland County to receiving waters named Sewickley Creek.

   NPDES Permit No. PA0218723. Sewage. Blacklick Valley Municipal Authority, P. O. Box 272, Twin Rocks, PA 15960 is authorized to discharge from a facility located at Blacklick Valley Municipal Authority STP, Buffington Township, Indiana County to receiving waters named Blacklick Creek.

APPROVALS TO USE NPDES AND/OR OTHER GENERAL PERMITS

   The following parties have submitted:  (1) Notices of Intent (NOIs) for Coverage under (1) General NPDES Permits to Discharge Wastewater into the Waters of the Commonwealth. The approval for coverage under these general NPDES permits is subject to applicable effluent limitations. Monitoring, reporting requirements and other conditions set forth in the general permit:  (2) General Permits for Beneficial Use of Sewage Sludge or Residential Septage by Land Application in Pennsylvania; (3) General NPDES Permit Authorizing the Discharge of Stormwater Associated with Construction Activities to Waters of the Commonwealth; (4) Notification for First Use Application of Sewage Sludge.

   The approval of coverage for land application of sewage sludge or residential septage under these general permits is subject to pollutant limitations, pathogen and vector attraction reduction requirements, operational standards, general requirements, management practices and other conditions set forth in the respective permit. The Department of Environmental Protection approves the following coverage under the specific General Permit.

   The EPA Region III Administrator has waived the right to review or object to this permit action under the waiver provision 40 CFR 123.23(d).

   The application and related documents, effluent limitations, permitting requirements and other information are on file and may be inspected and arrangements made for copying at the contact office noted.

SEWAGE FACILITIES ACT PLAN APPROVAL

   Plan Description:  The approved plan provides for extensions of the existing Charleston Township Authority collection system to include new gravity sewers with pump stations and low pressure sewers. Areas to be served include Charleston Road, Shumway Hill Road, Dart Settlement Road, Wetmore Hill Road, US Route 6, Whitneyville village and the Y-Drive-In area. The proposed Widger development south of the Village of Whitneyville will be provided sewer service as a result of this project. The sewage from this new collection system will be conveyed to and treated at the Borough of Wellsboro's Sewage Treatment Plant. The sewage treatment plant does have capacity for the 205 existing EDUs in this service area. The Charleston Township Authority does have capacity available per their treatment agreement with Wellsboro. The Borough has agreed to this proposal by co-adopting this plan on July 7, 2001

   Also, sewer extensions consisting of gravity and low-pressure sewers are being proposed for the 28 existing EDUs on Kelly and Lake Roads in the Hills Creek Area. This sewage is to be conveyed to the Hills Creek State Park sewage treatment plant. The Department of Conservation and Natural Resources has agreed to this plan and will be working with the township authority on a final agreement for services.

   The Department's review of the sewage facilities update revision has not identified any significant environmental impacts resulting from this proposal. Any required WQM Permits must be obtained in the name of the municipality or authority as appropriate.

   Plan Description:  The approved plan provides for construction of a combination gravity, force main, low pressure sewer extension to serve portions of Muncy and Muncy Creek Townships. The new line will traverse Fairfield Township but no portion of this municipality will initially be served. Service will be provided to the Halls Station area, the Lycoming Mall development and the Village of Pennsdale in Muncy Township and a portion of Muncy Creek Township between Halls Station and Muncy Borough. Treatment of the collected wastewater will occur at the existing Lycoming County Water and Sewer Authority wastewater treatment facility located along Tules Run in Fairfield Township. This facility discharges treated effluent to the West Branch Susquehanna River.

   The Department's review of the sewage facilities update revision has not identified any significant environmental impacts resulting from this proposal. Any required WQM Permits must be obtained in the name of the municipality or authority as appropriate.

LAND RECYCLING AND ENVIRONMENTAL REMEDIATION

UNDER ACT 2, 1995

PREAMBLE 2


The following final reports were submitted under the Land Recycling and Environmental Remediation Standards Act (35 P. S. §§ 6026.101--6026.908).

   Provisions of Chapter 3 of the Land Recycling and Environmental Remediation Standards Act (Act) require the Department of Environmental Protection (Department) to publish in the Pennsylvania Bulletin a notice of submission of final reports. A final report is submitted to document cleanup of a release of a regulated substance at a site where one of the Act's remediation standards. A final report provides a description of the site investigation to characterize the nature and extent of contaminants in environmental media, the basis for selecting the environmental media of concern, documentation supporting the selection of residential or nonresidential exposure factors, a description of the remediation performed and summaries of sampling analytical results which demonstrate that remediation has attained the cleanup standard selected.

   For further information concerning the final report, contact the Environmental Cleanup Program Manager in the Department of Environmental Protection Regional Office under which the notice of receipt of a final report appears. If information concerning a final report is required in an alternative form, contact the Community Relations Coordinator at the appropriate Regional Office listed. ARCADIS Geraghty & Miller, Inc., 3000 Cabot Boulevard West, Suite 3004, Langhorne, PA 19047 (on behalf of Penske Truck Leasing Company, LP, Route 10 Green Hills, Reading, PA 19603) submitted a Final Report concerning remediation of site groundwater contaminated with solvents and BTEX. The report is intended to document remediation of the site to the Site Specific standard.

   Carmeuse Pennsylvania Inc., North Londonderry and North Annville Townships, Lebanon County. BL Companies, 830 Sir Thomas Court, Harrisburg, PA 17109 (on behalf of Carmeuse Pennsylvania, Inc., Route 422 and Clear Spring Road, Annville, PA 17003) has submitted a Final Report concerning remediation of site soils and groundwater contaminated with lead, BTEX, PHCs and PAHs. The report is intended to document remediation of the site to the Statewide Health standard.

   P. R. Hoffman Materials Processing Corporation, Borough of Carlisle, Cumberland County. Earth Resource Associates, Inc. (on behalf of Hoffman Materials, Inc. 321 Cherry Street, Carlisle, PA 17013) submitted a Final Report concerning remediation of site soils contaminated with BTEX and solvents. The report is intended to document remediation of site soils to the Statewide Health standard.

   Aaron Rubin & Sons Company South Property, Susquehanna Township, Dauphin County. BL Companies, 830 Sir Thomas Court, Harrisburg, PA 17109 (on behalf of Aaron Rubin & Sons Company, 3401 North Sixth Street, Harrisburg, PA 17110) has submitted a Final Report concerning remediation of site soils contaminated with BTEX, PHCs and PAHs. The report is intended to document remediation of site soils to the Statewide Health standard.

   Caterpillar Inc., York Distribution Center, Springettsbury Township, York County. Earth Tech, Inc., Two Market Plaza Way, Mechanicsburg, PA 17055 (on behalf of Caterpillar Inc., 100 NE Adams Street, Peoria, IL 61629-3315) submitted a Final Report concerning remediation of site soils contaminated with BTEX and PAHs.
