 On December 12, 2023, the State Board of Nursing (Board) indefinitely suspended the license of Kathryn Blake, RN, license No. RN687924, last known of Norwood, Delaware County, retroactive to May 18, 2023, and assessed costs of investigation of $992.50 for being unable to practice nursing with reasonable skill and safety to patients and for violating a Board order.

 Individuals may obtain a copy of the final order at www.pals.pa.gov or by writing to Megan E. Castor, Board Counsel, State Board of Nursing, P.O. Box 69523, Harrisburg, PA 17106-9523.

 This order represents the final Board decision in this matter. It may be appealed to the Commonwealth Court of Pennsylvania by the filing of a petition for review with that court in accordance with the Pennsylvania Rules of Appellate Procedure. Individuals who take an appeal to the Commonwealth Court must serve the Board with a copy of their petition for review. The Board contact for receiving service of the appeals is the previously named Board counsel.
